HOSPITALIZATION

1. Health Management Before: Have regular check-ups but after 6

months treatment for HPN and DM becomes
noncompliant.
During: Follows orders of Physician to
maintenance due to relieved illness.

2. Nutritional – Metabolic Before: Eats 2 to 3 times a day but doesn’t

finish her whole meal. Usually eats pork and
beef due to allergies with chicken
During: Strictly follows diet as prescribed:
NPO, soft diet, low fat

3. Elimination Before: Voids freely, no pain in urination

During: Voids freely without assistance

4. Activity - Exercise Before: Walking/Commuting from house to

work
During: Can perform light exercise like
walking

5. Cognitive – Perceptual Before: Oriented, coherent

HEAD • normal scalp

• round and symmetrical

HAIR • black and white in color

• equally distributed

FACE • smooth
• no presence of nodules or masses

EYES • pupils equally rounded reactive to light

and accommodation with noted yellowish
discoloration of sclera
• no discharges noted

EARS • poor hearing

•auricles are symmetrical
• no discharge noted

NOSE • no presence of discharge and no flaring

noted

MOUTH • dry, pale lips

NECK • no tenderness
• flexes easily
• no nodules noted

CHEST • symmetric chest expansion with normal

respirations (RR: 20 cpm)

HEART • normal heart rate (CR: 98 bpm)

ABDOMEN • distended abdomen with noted

NAILS • long fingernails with poor capillary refill

(>3-4 seconds)

SKIN • yellowish in appearance
