Dear Math Lab Student,

Welcome to Math Lab at Sisters High School! This lab is designed for many different levels
of students from prepping for your next level of math, to brushing up on some missed skills,
to tutoring sessions with stronger math students, to prepping for the AP exam in the spring.

In order for us to best serve you, there will be a number of tools used to gather information
about you as a math student. We will do diagnostic testing, look at your transcripts, have
one-on-one meetings and have you complete some questionnaires. You will also be taking a
parent questionnaire home to have them complete. Please fill out the questionnaires as
completely and honestly as you can. This is extremely important in order for Math Lab to
work for you! All information gathered will be kept confidential and be used to better know
and serve you.

Below is a contract that must be signed by you and your parent/guardian if you wish to stay
enrolled in Math Lab. Please read carefully and understand your role and responsibilities in
this class.

The following criteria will be considered in order to determine the credit and grade that a
student will receive by completing a trimester of Math Lab.

 If a student has made sufficient progress in his/her math skills during the trimester,
the student will receive a grade of “Pass.” If sufficient progress has not been made,
the student may receive a “No Pass.” The instructor of Math Lab will determine if
the student has made sufficient progress to receive the “Pass.”
 A student that passes Math Lab will receive elective credit for Math Lab. The credit
earned in Math Lab will not be considered progress toward the SHS graduation
requirements in mathematics, with the exception of the following:
 A student may take Math Lab to recover credit in a previous math class that the
student received a No Grade. In this case, the student will get a credit in that math
class, but not a Math Lab credit.
 Similarly, a student may take Math Lab to improve a grade in a previous math
class, if the student did not make the grade prerequisite to enter the next math
course in the series (i.e. A student earned a D in Math 1B, but needs a grade of a C
or better in order to enter into Math 2). The instructor of Math Lab will determine if
the student has achieved at a level that meets the prerequisite and will assign the
grade accordingly. In this case, the student will get an improved grade in that math
class, but will not earn a credit in Math Lab.

These criteria will be reviewed and discussed with each student during the first two weeks of
Math Lab. The option that best serves the needs of each individual student will be
determined and agreed upon by the instructor, the student and the parent.

 I agree that there will be many students in the lab with various needs and abilities.
Thus, I must be polite, work hard, cooperate and always be a positive, contributing
citizen of the classroom in order for Math Lab to work for me and others. I also agree
that if I am not a positive, contributing citizen of the classroom that I may be
removed at the discretion of the instructor.
