Dear Parents & Guardians,

It is with great excitement that we invite your child to participate in an after-school tutoring and
mentoring program. In conjunction with volunteers from The Summit Church in Durham as well as Faith
Harvest Church in Durham; students will be able to participate in small group and/or one-on-one
tutoring & mentoring math and reading tutoring as well as mentoring based upon individual student
needs. In order to participate in this program, your child is required to have a signed permission form.

Tutoring & mentoring will take place on Mondays. Students will attend a session from either 2:45 – 3:45
or 4:00 – 5:00.

Please see the options below for which would pertain to your child’s needs and choose the one that best
fits your needs:

_____ Option 1 – Encore Program – If your child is already a member of the Encore After-school
Program, he or she may attend tutoring or mentoring during that time. If transportation is already being
provided for him or her, all you need to do is sign this form. Your child will then be picked up from the
Encore Program so that he or she can attend their tutoring session.

_____ Option 2 – Student Athlete – If your child is a student athlete, he or she may attend the tutoring
program during the required study hall block. If transportation is already being provided for him or her,
all you need to do is sign this form. Your child will be picked up from their study hall block in order to
attend his or her tutoring session.

_____ Option 3 – Transportation Needed – If your child is not currently staying after school, he or she
can still attend the tutoring program and receive transportation if you sign them up for the Encore After-
school Program. The application for the Encore program is attached to this invitation if you need it.
Simply sign this form as well as the Encore registration. Students can attend the Encore program
everyday or just on Mondays for tutoring based upon your preference.

We know that this program will have a positive impact on your child’s academics. Please sign below to
