School Screen ing Committee shall submit the list of applicants with the corresponding
documents to the Division Selection Commi ttees for kinderga rten/SPED, elementary(junior and
senior High schools through the Head, Personnel Services Secti on in soft and hard copy not later
than February 22, 2018.

3. New applicants and old substitute teachers shall strictly submit to only one (1) elementary or
secondary school head their application forms with the required documents. As mu ch as possible.
BSE Graduates and non-SSE with 18 units of Education shall be evaluated only for secondary.
4 . The application fonns shall be filed as far as practicable in schools located in the barangay or city
where the teacher-applicants reside.
5. An Applicant shall submit to the head of eleme ntary or secondary school a written application
supported by the fo llowing documer.ts in a long folder and each eam1arked for ease of
examination. lf any of these documents are not submitted, the Schools Selection Committee
may refuse acceptance of the application .
)> CSC Form 2 12 (Revised 2005) in two copies with the latest 2" x 2" ID picture
)> Certifi ed photocopy of PRC Certifi cate of Regis tration and License/renewed
license if expire
)> Certified photocopy o f rating obtained in the Licensurc Examination for Teachers/
Professional 13oard Examination for Teachers
)> Certified copies of Transcript of Records for baccalaureate course with
computed weighted average rating
)> Copies of service record, performance rating and school clearance for those
with teaching experience
? Certificates of s pecialized training, if any
)> Certi fi ed copy of the voter's ID
)> NBJ Cleara nce
)> Omnibus certification of authenticity and veracity of alJ documents submitted, sigued by the applicant.
ADDITIONAL REQUIREMENTS FOR SENIOR HS TEACHER APPLICANTS
)> Letter of intent which shall indicate the following information:
Statement of purpose/expression of interest
Subject group he/she intends to teach
Preferred school(s)
)> Certifi ed photocopy of Transcript o f Records with at least 15 units of special i7.ation in relevant
strand/special ized subject for Academ ic Track applicants
)> Certified photocopy of TESDA National Certificate (NC) and Trainers Mclhodology Certificate (TMC)
for TVL Track applicants
i-. Certified photocopy of Certification of Profi ciency/Recognition from recognized and respectable relevant
associations//organizations/guild.

6. The School Screening Committee shall verify all documents as to completeness and
authenticity . The committee shall issue a certification to the applicant that it has recei ved the
application specifying the documents that have been submitted in support of the application.

7. Public school teachers requesting for transfer to another assignment are not considered new
applicants and should not therefore be sub jected to these hiring guidelines.

8. As provided in Sec 26 (b) Paragraph 2 of RA 9293, teachers who have not practiced the ir
profession for the past five (5) years shatI be required to take at least twelve (1 2) units of
Education courses, consisting of at least six (6) units of context courses .

9. Teacher-applicants shall undergo the pre-employment mental/physical and medical examinations


required under the Magna Carta for Public School Teachers on ly a fter the Schools Division
Superintendent has approved their assignment orders/appointments and before they perform
their actual classroom teaching assignments .

10. To maintain the Registry of Qualified Applicants, all school heads are requested to receive
applications even if there is no available vacancy in their respective schools for future deployment
as replacement/substitute for teachers who will go on leave of absence.

11. The RQA should be ready by the last week of April of everv year so thal qua lified applicants may
be appointed before the opening of each schoo l year. The RQA shall be vo lid for a peri od of one
(!)school year.

12. Failure to subm it the required documents and to report for screening/evaluation on the
prescribed date will mean forfeiture of one' s right for employment in this Di vision.

13. Policy on hiring/recruitment of Teachers I a re subject to change upon issuance of a new


hiring guidelines from the Department of E ducation.

14. Particular attention is invited to the schedule of submission of pert inent documents to this
Office by district for proper implementation of t his undertakings.

15. Immediate dissemination and strict compliance with this memoran 111 is des ired.
