AFFIDAVIT

I, JASON MATO, of legal age, married, Filipino citizen,. with residence and postal address at No.31
Malvar St., Sitio Kamias 1, Mambugan, Antipolo City, after having been duly sworn to in accordance with
law, do hereby depose and state, THAT:

1. I was told by my aunt ESTRELLA A. PAJARO if I was interested to join the MARIKINA SILANGAN
TRANSPORT ASSOCIATION (MARSITA INC.) of which she was one among the members;

2. As according to my said aunt, they are recruiting more members so she accompanied me to the house
of a certain SPO2 ALVIN ABAR at his residence at Blk.42 Lot 33 Phase 2, Sta. Barbara 2, Silangan, San
Mateo, Rizal;

3. SPO2 Abar told me to pay an amount of P25,000.00 as membership fee for MARSITA in order to allow
me to join the group;

4. Hence, on June 5, 2016, I and my said aunt came back at the house of SPO2 Abar and I personally
handed to him the amount of P25,000.00 and the latter issued an acknowledgment receipt, a copy of
which is hereto attached and made an integral part;

5. Unfortunately, despite having paid SPO2 Abar for the said membership fee I was not able to use my
vehicle because he created another group U-HOP;

6. I made several verbal and written demand for the return/refund of the sum of P25,000.00 SPO2 Abar
failed, fai.: and refuse to pay the said amount to my prejudice;

7 This affidavit is being executed to attest to the truthfulness of the foregoing statements and for
whatever legal purpose this may serve.
