confirm under penalty of perjury and with individual and or full corporate responsibility that I have the
herein referenced and specified stock of Zimbabwe’s Banknotes of 50T under my sole control, that I
have full power to exchange then for United States Dollars and that I herewith irrevocably submit my
entire stock for recovery.

It is my understanding that I will be paid in USD and that the funds I receive will be covered with a Full
Global Immunity including a Global Tax Immunity and that appropriate accounts will be arranged as
per my request.

I hereby submit my stock as detailed below:

I also attach a copy of my CIS, Passport, POL and a list of 20 serials Banknotes and passcodes on the
Random Code Sheet for initial verification.

When I receive confirmation from you that my stock is approved for recovery I understand that I will
execute an Irrevocable Appointment to the Recovery Agent to complete this transaction on my behalf
and will without hindrance grant access to the stock for physical verification.

On completion of satisfactory physical verification I will, at a face to face meeting give you instructions
for disbursement to my accounts.
