B. Research Benefits for PT. Holcim Indonesia Tbk. Tuban Plant


1. The results of compliance measurements in this study can be used as basic data in
assessing one aspect of safety behavior that is compliance with the application of SOP
of Tools Grinding machine on workers at PT. Holcim Indonesia Tbk Tuban Plant
secara subjektif dan objektif.
2. The results of the measurement of knowledge in this study can be used as basic data in
assessing worker knowledge about SOP of Tools Grinding machine.
3. Personality measurement results in this study can be used as consideration in the
process of hiring maintenance workers, related to the type of personality that tended
comply with SOP of Tools Grinding machine.
4. Through research conducted at PT. Holcim Indonesia Tbk Tuban Plant, expected to
establish a good relationship between Public Health Faculty Airlangga University and
PT Holcim Indonesia Tbk Tuban Plant.

DESCRIPTION SHEET BEFORE RESEARCH


FOR RESPONDENT: GRINDING PARTS WORKERS PT. HOLCIM
INDONESIA TBK. TUBAN PLANT

To Research Respondents
My name is Dewangga Lazuardi, i was student on Master of Occupational
Health and Safety Program of Public Health Faculty of Airlangga University. I am
currently doing a research (thesis) with the title "Factors Associated with
Compliance Against Site Operational Procedure (SOP) Machine Tools Grinding
In Maintenance Worker Contractor PT. Holcim Indonesia Tbk. Tuban Plant”. For
that, I ask Mr. / Mrs to answer some questions in the next attachment. An honest
answer would be valid data and later can be related improvement suggestions

explanation of my research.
A. Research Objectives
The purpose of this research is to analyze factors related to compliance to
SOP Grinding Tools machine by maintenance contractor worker di PT.
Holcim Indonesia Tbk Tuban Plant.
B. Research Benefits for Research Resondents
Benefit of participation in this research is Mr / Mrs will obtain information
and knowledge about factors that play a role in compliance to Grinding SOP
Tool Machine in the form of giving booklets to workers.
C. Right to Self-Defeating
The participation of Mr / Br in this study is voluntary and Mr / Mr entitled to
resign at any time, without causing any adverse consequences for you.
D. Treatment of Research Respondents
1. Mr / Mr will get an explanation about the implementation of the research
2. Mr. / Mr will be required to sign the consent form to be an Informed
Consent if you express his / her participation is voluntary and non-
coercive, and there are no consequences whatsoever regarding your
work.
3. After signing the Informed Consent, Mr. / Mr is officially declared the
respondent of this research.
4. Mr. / Mr will be asked to fill out a questionnaire containing the
individual factors (age, education, years of work, knowledge, personality,
and motivation), occupational factors (supervision and availability of
facilities), organizational factors (training and communication), and
questionnaires adherence to SOP Grinding Tools machine.
5. Filling out the questionnaire takes approximately 60 minutes.

6. Furthermore, Mr. / Mr will be observed by the researcher for 15 minutes


or until grinding work is done, related to compliance to Machine Tools
Grinding SOP.
E. Data Privacy Guarantee
In this research, all data and information of Mr / Mrs identity are kept
confidential by not mentioning the identity of Mr. / Mr on the research report
and not showing the face (by covering the face) on the picture so that the
identity of Mr. / Mr can be kept confidential.
