A. Future health care trends


1. Name five major health care trends that will affect your practice in future.
This should be specific for your area of practice.
My Answers :
1) Increasing use of technologies in nursing care (telehealth/telenursing,
electronic health records, and tele–intensive care unit)
2) Increased partnerships and collaborations in taking care of patients
(inter professional collaboration/education)
3) Medical Tourism
4) Increasing complexity of patient care to holistic care (Palliative care)
5) Growing alternative therapy needs (religious approach with Murottal
holy Quran)

2015)(Syaiful & Nurwan, 2012)
2. What strategies can you put into place to prepare your workplace to meet
these challenges?
My Answers :
1) Trend 1 :
Nursing Technology Can be the Key to Improving Patient
Care, this technology will improve response times, increase accuracy
and ensure safety. In addition to helping patients, technology can also
help nurses and other clinical staff improve communication and
introduce greater efficiencies in clinical workflow which frees up
nurses and other staff to spend more time concentrating on delivering
the best possible patient care(Dorsey & Topol, 2016)(Middleton et al.,
2013)
Given the importance of using technologies, the nurses need
to improve skill how to use modern technologies and follow training of
medical staff to work with the new technology.
2) Trend 2 :
The Bussiness Case for Medication Safety stated about 7000
people a year from all over the world died from medical error. Based
on data from World Health Organitation (WHO), 80% of errors in
health services are caused by poor communication and understanding
of the patient's condition among medics. Bad teamwork will result in
overlapping services, conflicts, and errors in patient therapy(WHO,
2010)
Patient health problems that are complex can not be handled
by a single medical profession. patients have multi problems faced
require professional team to provide the best quality service. Patient
services with a multidisciplinary team-based approach, comprised of a
diverse group of specialized professionals(Afandi, 2012)
In my opinion, The nurses have to learn how to communicate
effectively, be teamwork, respect to each other (health profession).
And addition, inter professional colloboration should be applied in
university so that nurses can early understand about how to be
teamwork with different profession.
3) Trend 3:
Nurses can play an important role in educating patients on the
risks and benefits of traveling abroad for medical care. Nurses should
also understand the importance of their role when these patients
return. One of medical tourism barriers is language, so nurses should
master an international language to speak actively(AJN, 2017)
4) Trend 4 :
The effect Increasing aging population and cronic illness
enhance to need holistic care in healthcare. Palliative care is a current
trend of holistic care in Indonesia, Palliative care is a type of treatment
that specializes in terminal or chronic diseases where the approach
not only emphasizes physical symptoms, but also focuses on
emotional, psychosocial, and economic and spiritual aspects to meet
the need for improved patient quality of life. The goal of palliative care
is to improve the quality of life of patients through holistic care with
management of symptoms and relief of suffering (Kelley et al.,
2015)(WHO, 2014)
In Indonesia, palliative care services are still under
development so there is a need for specialized training for nurses in
palliative care(Anita, 2016)(Deli & Ana, 2014).
5) Trend 5
Religion can not be separated from human life. Of the various
cases there are many psychiatric people who are cured with a
religious approach. The Quran functions as an as-Syifa or medicine to
cure physical and spiritual diseases. A study that used holy Quran
recitation can decrease anxiety level on first active phase delivery
mother significantly and Quran recitation therapy is expected to be
used as an alternative intervention for managing anxiety level on first
active phase delivery mother(Ariadi, 2013)
Nurses need to know and aplicate religious approach to all
patient because of Remember, only in the remembrance of Allah the
heart becomes at peace(QS 13:28).
B. Evidence based practice
1. Which definition of evidence-based practise (EBP) fits best with your work
context?
Evidence based practice can be defined as the integration of
scientific evidence with nurses’ expertise and patient’s preference and
values in clinical practice to improve the outcome of patient’s
care(Oktiayuliandri, 2015).
2. Name three major criticisms of EBP as it is applied to nursing?
1) Are the research results valid and realiable?
2) Can the research results be confirmated?
3) Are the research results going to help nurses to take care of patients?
(Putradana, 2015)
3. What special cultural beliefs in health, illness and dying does your
community have which may conflict with evidence-based practice?
I am from southeast Sulawesi exactly Wakatobi island, so These are
according to wakatobi’s society beliefs and cultures :
1) For baby:
a. Newborn babies are banned out of the house .
b. Newborn baby is massaged at his stomach with warm hands or
objects to improve his digestive tract.
c. Newborn baby is recited with prayers from grandmother (shaman)
3 – 6 times to avoid congenital disease.
d. Fever in toddlers is believed by wakatobi’s society because new
teeth are growing
e. Toddlers are banned to eat many fish so that they have no
intestinal worms.
f. It is recommended for baby to cut fingernail or toenail on Friday.
g. When drizzle, it is forbidden playing out home because it will be a
headache.
2) For Girl: When menstruation, they should not be wetted her hair
3) For boy: after the circumcision (about 4-5 days), it is recommended to
swim at sea to recover quickly
4) Gestation period:
a. Drink coconut water, there are two versions
1) Prohibited because it can cause hypertension
2) It is recommended, as a ritual or attempt to get a beautiful and
pretty baby
b. Gestation period is prohibited outside the house at 6 pm
c. She must not eat the heart of bananas ( Banana Blossom), it is
believed to be the cause of congenital heart defects in children
5) Nifas (childbirth) Period :
a. Encourage eating boiled peanuts,it is believed to facilitate breast
milk
b. Do not eat meat, some types of fish, and coconut milk for 3
months since the first of childbirth.
4. How will you ensure sensitivity to these beliefs at the same time as giving
high quality nursing care?
In my opinion, as long as the beliefs are not contradictory or the
evidence is still in debate and not dangerous, my attitude I will give them
the opportunity to do what they believe while doing research about the
issue if it is necessary. However when it is dangerous and contradictory to
health, I gradually awaken by giving the right information and educating
the public.
5. What is your plan for ensuring that your clinical work and teaching is
based on EBP?
1) Reading the updated research results regularly
2) Attending a conference, seminar and workshop regularly
3) Building a culture of intellectual curiosity
4) Engaging in research activities
