Student Contract

I, _______________________________________ of the College of Human Kinetics at Cagayan State

University, Carig Campus do hereby enter to an agreement to obey and comply with the rules,
regulations, and implementing policies and guidelines of the University, effective upon enrolment. I fully
agree and promise that:

1. I shall wear the prescribe uniform and ID during school days and any day when required.
a. Academic Uniform and FS Uniforms (4th year level) during classroom-based subjects
b. University PE Uniform during PE Classes
c. Laboratory Uniform during outdoor and laboratory subjects
d. Organization T-shirt during wash day (Wednesday)
e. OJT Uniform for SWM Major and FS uniforms during Field Studies
2. I shall come to school on time during school days.
3. I shall secure and accomplish my clearance and attend my classes as officially scheduled by the
college concerned.
4. I shall not:
a. Get absent from my classes and other school activities and programs without justifiable
reason;
b. Smoke in the campus;
c. Gamble or bring gambling paraphernalia in the campus premises;
d. Take intoxicating drinks in the campus or come to school with the smell of liquor;
e. Take or use prohibited drugs or involve myself in drug pushing;
f. Carry and use deadly weapons inside the campus;
g. Assault any school personnel/faculty or any person in authority;
h. Forge any signature or falsify public documents or commit perjury;
i. Name-call, poke fun at, insult, satirize, or do any form of bullying to the university or any
person;
j. Commit vandalism or destroy public property and/or post commercial, political,
controversial. Litigious, subversive, seditious and contentious materials within the campus
premises;
k. Commit intellectual dishonesty like cheating and/or taking of, possessing of or passing on
examination leakages;
l. Commit theft/robbery in whatever stage;
m. Bring troublemakers and/or create any trouble or fight with any person or bully others
inside the campus
n. Commit immorality or maintain illicit relationship which include sexual infidelity and live-in
set ups and/or commit acts of lasciviousness or indecent acts like necking, petting, and
suggestive actuations done publicly within the university compound;
o. Join any fraternity/sorority or any organization not accredited by the university nor involve
myself in any activity not sanctioned by the university; (in case of those who are already
members)… that I am very willing to withdraw my membership with the
_________________________ fraternity/ sorority upon enrolment in the university;
 p. Join or commit any brutal and cruel acts as hazing or any form of conduct which tend to
threaten and endanger the health and safety of any person inside or outside the university
premises or which adversely affect the students’ suitability as member of the community;
q. Not allow anyone from using my school ID
5. I shall abide by the rules and implementing policies and guidelines under the university student
code of conduct and discipline which are not indicated herein;
6. If I fail to comply with the aforementioned policies and guidelines, I willingly agree without
reservation to the policy of forfeiting my enrolment in this university; and,
7. If I am caught and found guilty of violating any of this aforementioned policies and guidelines, I
will be willing to be suspended/dismiss/expelled from the university and/or accept whatever
disciplinary action the university may impose upon me.

STATE UNIVERSITY AT CARIG CAMPUS.

I promise to obey and comply with the policies and guidelines of the university as indicated in the
aforementioned STUDENT CONTRACT, should I break my promise; the university can impose the
corresponding disciplinary action or punishment upon me.
