How To Use
Start with the student profile page. This is helpful
information for the RtI or student study team who
may not be familiar with the student of concern.

It’s helpful to know if a student is younger, older,


or the same age as their classmates.

A variety of data measures show if the student


struggles in many areas (possible developmental
delay), or if they only struggle in one area (possible
learning disability or instructional gap).

You may have concerns about a student in more


than one area, but interventions need to be
focused. It’s like a science experiment where you
change only one variable and monitor for change.

Choose just ONE of the intervention pages. If you


move on to other interventions you can add on
additional pages.

This is where you define your targeted


intervention.

Choose an assessment that measures the


effectiveness of the intervention.

Take data at regular intervals to see how well the

intervention is working. You can add points in

between the vertical lines if you want to take data

over a longer period of time. The student’s score

before the intervention goes on the line labeled
“baseline.”

If the student is showing a reasonable amount of


progress then the intervention is working and
should be continued. If the child is showing little
or no growth then move on to another
intervention, a higher tier of the RtI model or
testing for special education.

As you carry out the intervention, record sessions on the included Intervention Implementation Notes form.
