ON THE JOB TRAINING MEMORANDUM

The purpose of this memorandum is to set forth a plan for training the student who is
enrolled in the Polytechnic University of the Philippines as a prerequisite for graduation from
the four-year course of the BACHELOR IN OFFICE ADMINISTRATIONin order that maximum
learning and job proficiency may be achieved by the student-trainee.

The student agrees to work and study diligently both when receiving business
experience and when attending school. The student also agrees to take advantage of every
opportunity to improve in skill, knowledge, and personality in order to qualify in his chosen
occupation. It is understood that the trainee will observe the same regulation, which apply to
other employees.

The employer agrees to assist the University in giving the students well-rounded training
by assigning the students to several phases of work station during the training period. Training
sponsors, usually supervisors, or department heads, or senior clerks, may be appointed to
direct the student’s job training in each of the work assigned. At the end of the training period,
the employer will complete the Training Evaluation Sheet, which gives an evaluation of the job
performance and personality of the student-trainee.

The coordinator will assist the employer in carrying out the continuous training of the
student. He will conduct the classroom instructions and correlate these instructions with the
on-the-job experience received by trainee at the work in order to discover to and to correct the
weaknesses of the student-trainee.

The parent or the guardian of the student has been informed about the progress and
permission has been secured for the student to receive the classroom instructions and the
business experience in the actual office.

The training period, covering the Second Semester of the school year 2015-2016 will
start on the date this memorandum has been approved and signed by the employer or his
representative. The student will report in the office from 11 a.m. to 3 p.m. This training
agreement may be terminated for just cause by given to all concerned parties.
