Mrs. Lisa Conner also stated that the last doctor to see her daughter was Dr, Allison psychological doctor at Ochsner Medical Center in New Orleans La, On August 17, 2016 I Investigator Garland Ward was reviewing the documentation surrounding this report, Based on the information obiained ffom the interviews and review of records om medical facilities it was determined that ad a significate medical history QJMIIMMB was prescribed as a tretinent for one of her Complaints.
