Sl Observations Remarks

1 Facility has no posted amfori BSCI code of conduct in their factory area.

2 No higher management responsible/ MR recruited for BSCI


3 Facility did not distribute BSCI code of conduct and TOI to its significant
business partners. Moreover factory has no system to monitor its supplier
regarding BSCI code of conduct.

4 a) The production capacity of the factory was not done properly.


b) There are No Business Contingency Plans in the factory.
5 The facility did not define a long-term goal comprising vision, mission and
objectives of the company in line with the BSCI values and principles to
protect workers.
6 Workers do not know about their legal rights and responsibilities.
7 No training or awareness provided to Works, Mid-Management (Marketing,
Merchandising, Commercial, Purchasing, etc.) and PC Members on BSCI
COC + TOI and supplier selection procedure. As well as other basic rights
as per law
8 Factory did not conduct satisfaction survey on the grievance mechanism.
9 Participation Committee (PC) was formed through selection process.
10 The formation of occupational health and safety committee does not
maintain as per law.
11 No Risk assessment ( transmittable and non-transmittal Diseases,
Machine and Section )
12 Training document was not update February-2018.(Orientation, Fire, First
Aid, Grievance, PPE, O.H.S )

13 The factory has training program for first aider based on monthly schedule
basis, however six month training is not provided for the first aiders as per
law.
14 Fire license and current floor layout plan is not included Medical room and
Child care room.
15 MSDS was not found Generator room.
16 Factory is keeping record of injury but no effective system in place to
analyze the injury & taken corrective action by identifying root cause which
will improve the occupational health and safety.
17 The workers in jacquard section found still working without ear plug.
18 Still pulley cover was not installed with boiler machine in boiler room.
19 The factory did not have an effective and functioning procedure or system,

21 Among Three electricians, One expires his certificate and remaining others
certificate expire date will be 8th April 2018.
22 a) Smoke detection system was not installed in PQC and Distribution area
b) Water hose reel does not cover PQC, Jacquard, Boiler, Chiller, and
Compressor and Generator area. Found no pressure in water hose reel.
C) PA system is not connected Yearn store and finish goods area.
d) Fire alarm systems not installed Distribution area and finish goods area.

23 Emergency exits sign does not work in 3rd gate in building one and Jacquard
inspection area.
24 There is no emergency treatment procedure.
25 a) Machine safety guard is not proper use.
b) Machine safety procedure is not proper maintained.

26 There is no canteen facility throughout the factory.


27 Hospital agreement expires on 15 January 2017.
28 Trained fire fighter from Bangladesh Fire Service & Civil Defense is 80.
Although this is not sufficient for 18% of total workers.
