Paul Nogier (1908-1996) sería hoy muy orgulloso de ver cómo muchos "hijos adoptivos" que
tiene en todo el mundo. En mi opinión, la oreja acupuntura es uno de los descubrimientos más
interesantes de los últimos 50 años.
Dos lecciones han sobrevenido de la experiencia de Nogier: la primera es que incluso un solo
médico, sin instrumentos caros y sofisticados, puede realizar una buena investigación, siguiendo
su instinto clínica y analizar rigurosamente sus observaciones. La segunda es que cada enfoque
revolucionario en el campo de la medicina tiene que luchar antes de ser aceptados por la
comunidad científica.Durante los últimos 20 años el gran aumento de los métodos
complementarios / alternativos ha traído una solicitud por igual cada vez mayor entre los
terapeutas y los pacientes para obtener información basada en la evidencia sobre su
eficacia.acupuntura de la oreja no es una excepción, y un conocimiento completo de la gama de
sus posibilidades debe ser traído a la luz.
Se puede decir libremente que todavía hay mucho trabajo por hacer tanto con el diagnóstico y la
terapia auricular auricular. En cuanto al diagnóstico todavía hay partes de la aurícula que tienen
que ser explorado a fondo, por ejemplo, el lóbulo de la oreja, la hélice y la superficie medial de
la aurícula. Imaginar que el sistema nervioso central está representada en el lóbulo de la oreja,
no significa que tenemos la clave para identificar sistemáticamente las áreas correspondientes a
partes específicas del cerebro. La simplificación expresa comúnmente por los investigadores
occidentales que una zona auricular es la representación de una estructura anatómica es
probablemente incorrecta, y debemos convencernos de que el mismo órgano puede tener varias
representaciones en el oído, como ha sido expresado por ambos practicantes chinos y los
médicos occidentales que practican medicina auricular.
Por lo tanto necesitamos una metodología específica para descubrir estas asociaciones con
métodos directos e indirectos. Los métodos directos, a través de modernas técnicas de imagen
como la resonancia magnética funcional, PET, etc., podrían evaluar qué estructuras del cerebro
se activan por estimulación auricular, ya sea en sujetos sanos o en pacientes con dolor u otros
trastornos neurológicos.
Los métodos indirectos, sin embargo, el uso de los procedimientos diagnósticos propuestos en
este libro, pueden ayudar al médico a identificar las áreas auriculares recurrentes asociados con
diferentes síntomas o síndromes. Este es, además, necesario porque a menudo no hay consenso
completo entre las diferentes escuelas en relación con las representaciones somatotópicos del
cuerpo.
La interpretación de estos múltiples grupos de puntos trae a la mente a alguien haciendo un
rompecabezas sin conocimiento de la imagen final.
Uno de los objetivos de este trabajo paciente y tal vez interminable es atribuir finalmente mucho
más importantes para el sistema nervioso autónomo, que hasta ahora parece tan
insuficientemente investigada. anatomistas contemporáneos no estudian la morfología de este
sistema y es poco probable que se dan subvenciones o financiación adecuados para dicha
investigación. En consecuencia los estudiantes de medicina siguen recibiendo la misma
información en unas pocas páginas de sus libros de anatomía como lo hicieron hace 20-30
años. El sistema nervioso autónomo, sin embargo, es esencial para todas las funciones de
nuestro cuerpo y el diagnóstico auricular nos da la oportunidad de examinar la Concha, que se
debe recordar es la única zona del cuerpo donde el décimo nervio craneal sale a la superficie.
En cuanto a la terapia auricular, hay mucho trabajo por hacer en los siguientes aspectos:
En primer lugar, a pesar del gran interés en la acupuntura de la oreja en todo el mundo, todavía
hay una falta de información basada en la evidencia acerca de los campos en los que el
tratamiento auricular podría aplicarse de forma más ventajosa. Algunos se ha encontrado
evidencia para el tratamiento de trastornos de dolor, como el lumbago , dolor cervical y la
migraña . Además se ha encontrado evidencia para el tratamiento de la adicción a las
drogas y la obesidad , así como para la ansiedad y el insomnio . Terapia auricular para el
tratamiento de los trastornos psicosomáticos parece ser prometedor y debería, en mi opinión se
propone como una terapia complementaria en asociación con psicofármacos.
Otra combinación terapéutica interesante podría ser la inclusión de acupuntura de la oreja en los
programas de rehabilitación, para reducir el número de sesiones y el costo total de la
rehabilitación.
El segundo aspecto importante de la terapia auricular, que aún no ha sido tenido en cuenta
adecuadamente, es la medida del efecto terapéutico de la estimulación no invasiva de la aurícula
tales como el masaje, perlas magnéticas, las semillas de Vaccaria, la estimulación eléctrica y de
láser, etc. este es un tema importante porque muchos terapeutas no son médicos y en varios
países no pueden insertar agujas. La peculiaridad de la aurícula desde el punto de vista
neurofisiológico parece apoyar la hipótesis de que la luz, la estimulación no invasiva puede tener
un efecto terapéutico significativo. Hay una necesidad urgente para hacer frente a estas
cuestiones con el fin de colocar la acupuntura auricular definitivamente entre los métodos
terapéuticos complementarios basados en evi-denced.

Paul Nogier (1908-1996) would today be very proud to see how many 'adoptive children' he has
around the world. In my opinion, ear acupuncture is one of the most intriguing discoveries of the
last 50 years.
Two lessons have ensued from Nogier's experience: the first is that even a single physician,
without expensive and sophisticated instruments, can perform good research, following his
clinical instinct and rigorously analyzing his observations. The second is that every revolutionary
approach in the field of medicine has to struggle before being accepted by the scientific
community. Over the last 20 years the great increase in complementary/ alternative methods
has brought an equally increasing request among therapists and patients for evidence-based
information on their efficacy. Ear acupuncture is no exception, and a full knowledge of the range
of its possibilities needs to be brought to light.
It can be freely stated that there is still much work to do with both auricular diagnosis and
auricular therapy. As regards diagnosis there are still portions of the auricle which have to be
thoroughly explored, for example the ear lobe, the helix and the medial surface of the auricle. To
imagine that the central nervous system is represented on the ear lobe does not mean that we
have the key consistently to identify the areas corresponding to specific parts of the brain. The
simplification expressed commonly by Western researchers that one auricular zone is the
representation of one anatomical structure is probably wrong, and we should convince ourselves
that the same organ can have multiple representations on the ear as has been expressed by
both Chinese practitioners and Western physicians practicing auriculomedicine.
Therefore we need specific methodology to discover these associations with direct and indirect
methods. The direct methods, through modern imaging techniques such as fMRI, PET, etc.,
could evaluate which structures of the brain are activated by auricular stimulation, either in
healthy subjects or in patients with pain or other neurological disorders.
The indirect methods, however, using the diagnostic procedures proposed in this book, may help
the clinician to identify recurrent auricular areas associated with different symptoms or
syndromes. This is furthermore necessary because there is often no complete consensus
between the different schools concerning the somatotopic representations of the body.
The interpretation of these multiple clusters of points brings to mind someone doing a jigsaw
puzzle with no knowledge of the final picture.
One of the aims of this patient and perhaps endless work is to attribute finally much more
importance to the autonomic nervous system which thus far seems so insufficiently investigated.
Contemporary anatomists do not study the morphology of this system and it is unlikely that they
are given adequate grants or funding for such research. Consequently medical students still
receive the same information on a few pages of their anatomy books as they did 20-30 years
ago. The autonomic nervous system, however, is essential for all functions of our body and
auricular diagnosis gives us the opportunity to examine the concha which it should be
remembered is the only zone of the body where the tenth cranial nerve comes to the surface.
First, despite great interest in ear acupuncture throughout the world, there is still a lack of
evidence-based information about the fields in which auricular treatment could be applied more
advantageously. Some evidence has been found for the treatment of pain disorders such
as lumbago, cervical pain and migraine. Further evidence has been found for the treatment
of drug addiction andobesity as well as for anxiety and insomnia. Auricular therapy for the
treatment of psychosomatic disorders seems to be promising and should in my opinion be
proposed as a complementary therapy in association with psychoactive drugs.
The second important aspect of auricular therapy, which has not yet been taken into
consideration adequately, is the measurement of the therapeutic effect of non-invasive
stimulation of the auricle such as massage, magnetic pearls, Vaccaria seeds, electrical and laser
stimulation, etc. This is an important issue because many therapists are not medical doctors and
in several countries are not allowed to insert needles. The peculiarity of the auricle from the
neurophysiological point of view seems to support the hypothesis that light, non-invasive
stimulation may have a significant therapeutic effect. There is an urgent need to face these
issues in order to place ear acupuncture definitively among the evi-denced-based
complementary therapeutic methods.
