AFFIDAVIT OF LOSS

We, MELANIA D. DELMO and JOSE JINNY M. DELMO,

both of legal age, Filipinos and residents of Barangay Buhang, Paho,
Iloilo City, under oath, depose and say that:

We are the Vendees of a residential lot of Sta. Lucia Realty &

Development, Inc., described as follows:

Transfer Certificate of Title No. T-126225

A parcel of land (Lot 11, Block 70, of the Subd.

plan, PCS 06-002462, being a portion of Block 1-56. Road
Lot 1-21, all of (LRC) (Pcs-18292), situated in the
Barangay of Bito-on, Buhang & Balantang, District of
Jaro, City of Iloilo, Island of Panay. Bounded on the SW.,
along line 1-2 by Lot 9 of Block 70 of the Subd. plan; on
the NW., along line 2-3 by Road Lot 65; on the NE., along
line 3-4 by Lot 13, and on the SE., along line 4-1 by Lot
12, both of Block 70 of the Subd. plan. Beginning at a
point marked “1” on plan., containing an area of Two
Hundred Thirty Four (234) square meters, more or
less.

Marked and attached hereto as Annex “A” to be taken as an integral

part hereof is a true photocopy of the Deed of Absolute Sale making
us Vendees of the above described property.

That by virtue of the aforesaid sale, Sta. Lucia Realty &

Development, Inc. had already turned over to us the owners copy of
the Transfer Certificate of Title No. T-126225 covering the above
described property.

That we have misplaced said Owner’s Certificate of Title and

despite diligent efforts exerted to look for it, the same could no
longer be found or located.

We are executing this affidavit in order to attest to the truth of

the above-stated facts and to inform the authorities concerned that
we lost said Owner’s Certificate of Title in good faith and request the
Registry of Deeds to issue us a new Owner’s Certificate of Title in lieu
of the lost one.
 IN WITNESS WHEREOF, we have hereunto set our hands
