Program Goals

 Complete head to toe assessment on patient, as well
as focus assessment based on diagnosis
 Measuring patient outcomes based on specific
patient goals
 During my shifts I have witnessed different dressing
for pressure ulcers and lacerations, I have removed
staples from a post op patient, flushed peripheral IV
lines and NG tubes, as well as operated feed lines for
NG tube
 One of my patient required bilateral lower leg
dressings using inadine, non-sterile
 Administered medication with the supervision of my x
preceptor, PO and through NG and PEG tube
 Based on pre-clinical research, preforming evidence
informed interventions which will best suit my
patient, to ensure I am able to provide information to
patients on what medication they are receiving, I
make sure I look up any medications the patient is on
that I do not know prior to dispensing the medication.
This ensures I am accountable for medications I give
and am informed as a student nurse.

 During each shift, I demonstrate therapeutic care and
communication by completing charting and report
back information gathered such as vitals to my
assigned nurse; additionally, when I let a patient
know I will go get something for them or come back
to check on them, I make a conscious effort to do this x
in a timely manner
 To create a culturally safe environment I have
reflected on my own culture, views and attitudes. I
can recognize stereotypes and avoid using them as
they can be offensive and not true.
 Using verbal and nonverbal forms of communication
to communicate effectively with members of the
health care team and patients.
 I try to be confident when talking to patients, as well
as I keep an open-mind, I use active listening and
giving time for patients to finish their thoughts if it
takes a while to be communicated. I try to use good
body language that is appropriate and eye contact.
 I have struggled in some situations on what to say to
patients as it is sometimes hard for me to
communicate with older patients and takes patience.
In other cases I am intimidated by some nurses
working on the unit and find it hard to approach them
because I am nervous.
 Through previous classes and especially
pharmacology, pathophysiology and acute care
classes I can apply empirical knowledge based on

 Through maintaining a positive attitude and the way I

 I believe through each SIM, class, and clinical shift I
am gaining personal knowledge by experiencing
things first hand and becoming more aware. This
helps me provide personal meaning to things I may
have learned in class, so I can better use it in practice
 Though in this placement I haven’t had any major
ethical issues, if I did I would use my gained ethical
knowledge and call on the more experienced ethical
knowledge of other nurses and staff
 Open communication with staff on unit and clinical
preceptor to make sure I am contributing to a safe
and open environment
 Practice within my nursing student scope of practice
and adhere to hospital and school policies.
 Ask for assistance from nurses on the unit when
needed and seek help from other nursing students.
 Check with preceptor or assigned nurse if I have any
concerns with vital signs found.
 I believe I have met this objective by completing my x
first reflection. For my first reflection I focused on an
event that stuck out in my mind to patient and nurse

safety, which happened on my 2nd shift. I focused on

problems I saw with the situation and how I think it
could have changed for better or worse. By reflecting
I was able to learn from the past if I ever come across
a similar situation.
