At the time of the call, Tamika told dispatch that her and her staff have noticed different "marks" on the child throughout the last week. Tamika also requested an ambulance because she has noticed that the child was bleeding from her ear and had sustained a head injury. I was dispatched to the facility. Upon arrival, Euclid Fire Department rescue squad #1341 was on scene tending to the child, I identified the child as Aniya M. Day-Garrett. I observed Aniya before she was transported to the hospital and she had dried blood inside of her left ear, large scabs covering the majority of her nose, and some abrasions to her scalp between her braids. Aniya was shy around strangers and would not speak with me about what happened, Aniya was transported to Euclid Hospital via EMS without incident, and the paramedics left her in the care of Physician Assistant Michelle Mochler and the Emergency Department staff. Michelle Marshall, an employee of the child care facility, followed the rescue squad in her personal vehicle to the hospital. 1 spoke with Tamika and she reported that Aniya lives with her mother Sierra Day at 24451 Lakeshore Blvd., #119, Tamika spoke with Sierra on the phone and advised her that Aniya was being transported to the hospital ‘Tamika reported that she took over ownership of the facility in April of 2017. On Monday of this week, May 15th, 2017, Sierra brought Aniya into the facility to drop her off. At that time, Aniya had fresh scrapes to her nose. Sierra told staff that Aniya fell while playing at the park. However, when staff spoke with Aniya, she (old them that mommy had pushed her down at home and her nose got hurt on the carpet. Aniya also told them that "mommy keeps saying that it happened at the park". Per policy, Tamika documented Aniya's injury in an internal incident report known to them as a "Child Observation Form". When doing so, Tamika noticed a stack of similar incident reports in Aniya's file, dating back to 2015. The previous reports were made by the previous owners of the facility. A few days later on Thursday May 18th, 2017, Sietra dropped Aniya off at the facility. That is when staff noticed the dried blood in her ear and the abrasions on her head. Tamika completed another incident report and contacted poli ‘Tamika provided me with copies of all of the previous incident reports made concerning Aniya. I reviewed the reports and summarized them as follows: September 17th, 2015 - Bruise to right side of face, looked like a hand print. October 22nd, 2015 - Bruise to right side of face, looked like a hand print. Child eried most of the day September 12th, 2016 - Bruises on face, eye, and arm, Mother stated that Aniya had fallen out of bed and she removed some bandages from where Aniya had gotten shots, September 21st, 2016 - Bruises on her face, under her eye and on her eye lid. Aniya stated that mom did it. October 25th, 2016 - Bump on head. Aniya was crying. December Ist, 2016 - Aniya had a bruise under her right eye and was crying. Aniya stated that mom had hit her. Mom stated that it happened at Applebee's. December 2nd, 2016 - Bruise on left? Aniya stated that mom has been hitting her. Aniya stated that mom hit her. December 27th, 2016 - Bruise on forehead. Aniya stated that mom and dad did it. Mommy hit me. January 18th, 2017 - Aniya came (o school with several bruises on the face, arm, and back. Aniyah has a raised bruise on her left upper forearm. When asked about it, Aniya stated that her mommy had scratched her February 10th, 2017 - Aniya has a dark colored bruise under her left eye. Aniya stated that mom did this. Staff had noticed bruises on her everyday when she comes in. March 14th, 2017 - Right side of lip. Busted lip. May 15th, 2017 - Scrape on top and under nose. Mother said child fell at the park. Child says mommy pushed her down at home. May 18th, 2017 - Right side of head and ear. Swollen and dried blood. Copies of these incident reports were attached to this report. Tamika and Michelle completed written statements, Tasked Tamika about Aniya's behavior while she is at the childcare facility. Tamika told me that Aniya constantly wants to be held and hugged. She stated that she constantly asks, "Am I being good?" Sierra came (o the child care facility while I was on scene. She was advised that Aniya had been transported to the hospital via ambulance. Sierra told me that when she dropped Aniya off, she did not notice blood inside her ear or abrasions to her scalp. Sierra implied that the injuries were caused by the childcare facility denied physical abuse in her home. I asked Sierra who Aniya had been with recently and she told me " ‘grand daddy”. She stated that she was with him until about 2300 hours on May 17th, 2017. Sierra then picked her up and brought her home. I asked for his information and she identified him as Michael Lared. I asked for a phone number for him, but she stated that she did not know it and that she only communicates with him through facebook. I asked her where he lives and she would only tell me "the west side", She then went on to tell me that he is not really her grand daddy but is a friend's dad. 1 asked what the friends name was, and she stated Dashiaa Marie, However, she was unable to provide me with any further information on Dashiaa. Sierra then left to go the hospital. [attempted to locate further information on Michael Lared and Dashiaa Marie, but was unable to locate any matches within LEADS T responded to Euclid Hospital. I spoke with the hospital social worker, Jessica Jenkins, and she advised me that she had contacted Cuyahoga County Children and Family Services. A social worker was on her way to the hospital. I spoke with nursing staff, and they advised me that they had evaluated Aniya, and did not locate any other scars, marks, bumps, or bruises on her body. I photographed the abrasions to Aniya’s scalp, the scabs on her nose, the dried blood in her tight ear, and a scratch the on the right side of her chin. Aniya kept complaining of pain and discomfort on the right side of her face. I spoke with Sierra further about the incident in the waiting room of the Emergency Department. It was decided that Sierra and Aniya be seperated until the county social worker arrived. She told me that Aniya has told her that they hit her, but she did not take it seriously because Aniya lies. I gave Sierra the opportunity to complete a written statement to be attached to the report, but she refused stating that she was too stressed at this time. She ‘was given a statement form to take with her, should she wish to complete it at a later time. Sierra agreed to sign a "Cleveland Clinic Authorization to Disclose Health Information Form’. County social worker Tawana James arrived at the hospital. I passed on the information I had and provided her with copies of the childcare facilities incident reports. She provided me with county intake number 38988833, Tawana spoke with Aniya and Aniya spoke to her. Aniya told her that her mommy hits her and. hurts her.
