Dear Sir / Madam,

Re: Ms Nicole Smith.

I am writing to refer Ms Smith, an 18 –year-old single women, who was admitted to our hospital on
9th September 2009 for delivery her first baby. She had emergency caesarean section on 10/09/09
followed fatal distress and inadequate labour progress.Ms Smith and her baby are due to be
discharged today.

During hospitalisation, she has recovered well and the wound has healed and free of infection.
Although her blood loss is minimal now but receiving fefol tablet and vitamin C as treatment. The
baby is male and breast feeding that his birth weight was 4.4 Kg on 10/09/09.

In view of Nicole’ circumstances, she has had no contact with her husband during the previous 6
month and also has no family in Brisbane. She lives in a rental share flat with 2 other women and has
not any income but she received some money from her parents recently. Nicole is non smoker and
no drink alcohol or other drug.

It is important to know, she is reluctant to continue breast feeding and like to change to bottle
feeding. She has no self-esteem for bathing and looking after her baby .Today her son is 4.1 Kg with
good feeding as well as has no any sign of jaundice.

I would appreciate it if you could give some advice and persuade her for breast feeding and also
arrange regular home visits regarding assistance for Nicole and her baby should you require any
further information please do not hesitate to contact me.
