Amount Granted: ONE HUNDRED FIFTY THOUSAND PESOS (P 150.000.00)

For value received, I promise to pay GJ TALAVERA LENDING INC. or order at its office at SAN FERMIN
CAUAYAN CITY, ISABELA THE SUM of ONE HUNDRED FIFTY THOUSAND PESOS (P 150,000.00) Philippine
Currency, including renewals, extension or roll-over thereof with interest at the rate of 3 % per month from the
date hereof until fully paid according to the payment schedule as hereto attached.

The interest rate on this note shall be subjected to regular review by GJ TALAVERA LENDING INC. In case
of a change in the prevailing market rate for the similar type of loan as determined solely by GJ TALAVERA
LENDING INC. & which determination shall be conclusive on me. GJ TALAVERA LENDING INC. may at any time from
time to time, increase or decrease the interest rate agreed upon in accordance with the said prevailing market
interest rate, said increase or decrease shall take effect upon notice to me. Should I disagree to the upward or
downward change in the interest rate. I shall have the right to immediately prepay the loan within thirty (30) days
from receipt of notice from GJ TALAVERA LENDING INC. of the adjustment in interest rate, otherwise, I shall be
deemed to have agreed on the interest rate effective as of the date notice.

In case of default, I bind myself to pay GJ TALAVERA LENDING INC. a sum equivalent to FIVE (5%) percent
per month of the principal amount of loan as penalty.

Non-payment of the stated installment including interest & penalties thereof shall render the entire
balance including accrued interest unpaid and FIVE (5%) percent per month penalty immediately due & payable.

In the event that this note is placed in the hands of a lawyer for collection, I shall pay at least twenty five
(25%) percent of the amount due on the note but in no case shall not be less than FIVE THOUSAND PESOS (P5,000)
as Attorney’s Fees plus interest exclusive of all cost & fees incurred by the GJ TALAVERA LENDING INC. & as
stipulated on the contract or other agreement, if any.

To guaranty the payment of the aforesaid loan, I hereby issue the following checks:

Pursuant to the provisions of the loan agreement and this note, if any, the consent herein granted is
recognized and acknowledged by me as a waiver, to all intents and purposes, of whatever right I may have. And
finally, this note may be enforced judicially and extra judicially without further demand.
