Flowchart on Triage

Start

Upon entry to the Emergency Room (ER):

 Assess patient in 2-5 minutes

 Register patient in the Triage logbook and ER Form with the
following data:
 Name of Patient (Surname, First Name,
Middle Name)
 Chief complaint/s
 Age
 Date and Time of Consult

Measure vital signs (RR, PR, BP, Temperature and mental status

Determine clinical urgency based on 3-point Triage Scale

Urgent Non-Urgent

Wait for further Category? Refer to OPD

assessment and/or
observation

Emergent

Document details of the triage assessment in the Triage

Form

Notify doctor at the respective Clinical Departments END
