Esse foi o panorama histórico que gerou a demanda pela procura por
diagnósticos psiquiátricos padronizados com algum grau de evidência na infância e
adolescência, culminando na criação e a ampla utilização dos critérios diagnósticos
operacionais (FEIGHNER, 1972) (ROBINS & GUZE, 1970) (RUTTER, 1965). Assim
foram formulados os critérios diagnósticos para quadros psicopatológicos na infância
e adolescência como os conhecemos atualmente, com uma preocupação focada na
procura de maior especificidade e alguma comprovação, a partir das publicações do
DSM-IV em 2000 e CID-10 em 1996 (RUTTER, Michael, 2011, p. 27).
Fenomenologia-Estrutural
Casos clínicos:
Pais contam que sempre foi muito dócil, bem humorado, sorridente. Sempre
foi muito distraído, envolvia-se em acidentes facilmente por não prestar atenção no
ambiente. Perdia com frequência objetos, mesmo brinquedos ou jogos eletrônicos
de que gostava muito.
Conta que gosta muito de pensar, que sua cabeça cria muitas histórias e que
é difícil prestar atenção na aula. Tem muitas histórias "inventadas”, mas não
apresenta intenção espontânea de relatar o conteúdo. Essas histórias, depois de
insistir em saber sobre o que inventava, mostram-se ricas, cheias de personagens,
ações bem elaboradas, adequadas para seu perfil cognitivo e cronológico. Deixa
claro que tem consciência de que deveria prestar mais atenção nas aulas, mas
conta que era muito difícil controlar a sua imaginação.
Conclusão:
A psiquiatria como um todo vive um momento de reflexão sobre seus
instrumentos de reconhecimento psicopatológico e sobre a prática clínica. Essa
reflexão é ainda mais relevante no segmento da Psiquiatria da Infância e
Adolescência pela sua imaturidade cronológica e campos de conhecimento pouco
explorados.
