AFFIDAVIT

I, RAMON D. ALVAREZ JR., of legal age, married, Filipino, with residence at

26 Lapu-Lapu St., Gonzaga Subdivision, Brgy. Villamonte, Bacolod City, Negros
Occidental, after having been duly sworn to in accordance with law, do hereby depose
and state the following:

1. That I am the registered owner of RAJR WATER REFILLING STATION as

evidenced by the DTI certification attached herein;
2. That our operation commenced last December 2017;
3. That on December 7, 2017, we complied with the requirement of Water
Bacteriology Analysis Test Report and Physico-Chemical Analysis Report on all
our water products - all conducted by Negros Prawn Producers Analytical &
Diagnostic Laboratory (copies of said Analysis Reports are herein attached);
4. That we are currently applying for a Sanitary Permit to Operate from the City
Health Office of Bacolod City;
5. That our failure in subjecting our Finished Products for Water Bacteriology
Analysis and Physico-Chemical Testing was by reason of inadvertence,
excusable neglect and inexperience on all the rules and regulation which the
business is subject into;
6. That the omission was not intended to defraud the government or the general
consuming public;
7. That as neophytes in the Water Refilling Business, we are still in the process
of learning all the nuances of the business and perfection of the said endeavor;
8. That we undertake to subject all our Products to all the required Tests moving
forward, and comply with all the rules and regulation relative to the said
business;
9. That I am executing this affidavit to attest to the truth of the foregoing and for
whatever legal purposes that this statement may serve.
