Date & Time: March 21st from 4-4:30 p.m.
Audience: East Central Health District Employees at the Richmond County Location

Objectives

Affective:
1. The participants will discuss 3 concerns about the 5-Week Work-site Wellness Program
on a participant feedback form.
2. The participants will discuss their positivity towards the program in a question and
answer session.

Introductory Statement

Let’s kick chronic disease! With this presentation, we got you covered on how you can move
away from chronic disease by introducing our 5-week work-site wellness program.

Introduction About Me
 I am from Augusta.
 Attend Georgia College in Milledgeville, GA
 Senior Public Health Major
o My Public Health interests are comprehensive sex education, women’s health,
diabetes and heart disease.
o Plans on becoming a Certified Nurse Midwife
 Graduate May 12th
 Plan on going to nursing school
o Eventually want to be a Certified Nurse Midwife
 Completing my internship here at the East Central Health District
o Chronic Disease Department
 Work-site wellness is a part of my project for class
o Will be included in my online portfolio to show the work that I have done along
with this presentation

 Accidents/Unintentional Injuries
 Data is from 2011 and 2015 and there are not many changes in these
rankings as to what causes the deaths.
 The amount of deaths is increasing within these deaths

 Week 5: Keeping Up the Momentum with Your Progress

 Point System Sheet and rules
 The points are ranged from 1-5.
 Coming to the meetings will result in a double of the maximum
points (10 points).
 You only can get credit once a day for a task completed. Example:
You drank 32 oz. of water. You only get 1 point that day for
completing that task.
 You earn points for do some of the unhealthy behaviors too.
o It is not meant to punish you. You try and work hard but
fall short sometime.
 You still get points for:
 Cheat meals/snacks
 Partially healthy meals
 Failing but you tried
o You will not get as many points but it is still points for you
to earn.

 The presenter will show an image within the first 3 slides of the PowerPoint with no
explanation and ask the audience to explain/identify it.
 The audience must justify why they said what they said about the picture.
 The goal is to have the minds thinking about a healthier lifestyle based off the picture
chosen.

 Allow the faculty and staff to ask questions
 The faculty and staff will complete the Participant Feedback form
 Allow the faculty and staff to sign-up for the program
 Distribute flyers to those who are not on the email list
o Other handouts will be given out as well.
 Might have more time if the presentation gets done early

Time Allotted

The employees are only allowed to be away from their desk for 30 minutes as it relates to their
built-in exercise minutes for the day.
