Judie is able to apply the concepts learnt in theory and skills labs and apply them to the clinical setting - administering medications,
transferring clients and simple dressings. She is able to look at the client in a holistic manner and provide care based on her assessment.

Judie is able with guidance to document her assessments, vitals and care provided, using appropriate clinical terminology in the computer
charting system on her clients. She communicates with her co-assigned nurse and other members of the inter-professional team. She is
able to use technology and conventional methods of communication so that they are aware of what is happening with the client,she
ensures that she documents pertinent information and also communicates it to her co-assigned nurse and has also learnt to communicate
with other members of the inter-professional health team. Judie attentively listens to her clients which is a therapeutic communication
technique.

when I had a resident that was deaf in one ear, hard of hearing in the other and blind. During his meal, I spoke loudly and clearly into the ear that

could hear and explained what what in front of him using numbers ( like on a clock) as a reference.

because he was itchy, so I informed him that if he kept on exposing his open wound it could get infected and take longer to heal. The resident left the

Judie has built relationships with her clients and is able to communicate with them despite their diagnosis' of dementia. She has learnt
how to prioritize care that she is going to provide for them, she gives the clients options when she is providing care and does not impose

interact with them,

moving a resident in bed, and the advice she gave me has been used ever since.

Judie has been the leader of the group. Judie has developed her leadership role by taking charge of her clients care, she plans their care
and will collaborate or consult with other members of the health team to ensure that she is following nursing best practices or the policies
of the facility. She plans the clients care and will alter it based on the clients choices. Judie completed a peer review and submitted one
for her peer as well. She ensures that all her peers are aware of the plans for conference, assignments to be submitted etc. She can
redirect clients when they refuse to eat or drink and encourage them to do what needs to be done to provide them with care.

when looking at the client as a whole to guide her practice. She uses knowledge and skills learnt in school when assessing her clients,
prioritizing needs and outcomes and to help her to prioritize care and interventions in order to manage workloads and her time efficiently,
(gathering all equipment required to provide care to a client, organizing care so that she can provide care equally to her clients. She is able
to modify the plan of care that her client is receiving).

Judie has interacted with some members of the health team such as pharmacy, other nursing members and the physician and she has
worked with her peers and co-assigned nurses as part of the team in providing care to her assigned as well as a other clients, she will
assess and collaborate her client, seek assistance when she needs help, to achieve the expected outcomes that were agreed upon.

the health care system has certain standards of cleanliness and while at clinical I was able to keep the work stations (like the medication tables) clean

after each use.

Judie always watches over her clients so that they do not come to any harm, she will monitor the client if they are confused and pulling at
any clothing. She reviews the units policies and procedures prior to completing a skill. She was quick to pick up on the computer charting
system used at the Veteran's Centre. Judie was always seeking out new opportunities of learning. She also would assist the clients.

the guidance with the freedom to work on our own this semester has taught our whole group to become more independent in the field. Strategies for
continued growth include keeping up with my readings, and to continue seeking as many learning opportunities as possible to practice skills.

Judie has grown as a nurse in this placement, although she did not get to experience many nursing skills, I believe that she will master them during
her next practicum if she is place in an acute care setting. I encourage her to continue building on her skills, especially her leadership and critical
thinking skills. For those skills that she was not able to practice in the clinical setting, I encourage her to review and practice them frequently.
