MOBILIZATION WITH PROVIDING THE POSITION OF THE PLATE

Aim :

1. Maintain body alignment


2. Reduce complications due to immobilization
3. Reduce Increase comfort
4. Possible injury to nurses and clients
5. Reduces the possibility of persistent pressure on the body due to a settled position

Indication:

1. Patients who experience paralysis both hemiplegi and paraplegi


2. Patients who experience weakness and postoperative
3. Patients who experience treatment (immobilization)
4. Patients who experience decreased consciousness

Preparation :

1. Explain to the client the intent and purpose of doing the mobilization action to the lateral
position
2. Wash hands before taking action to limit the spread of germs & micro organisms
3. Remove any obstacles so that the nurse moves freely
4. Prepare the equipment in need.
5. Keep client privacy

Tools preparation :

1. One arm support pillow


2. One leg support pillow
3. The back support pillow

Ways of working :

1. Raise / remove the bedside border rail on the side where the nurse will mobilize
2. Ensure the patient's position in the center of the bed, the position of supination easier
when done lateral mobilization
3. Nurse takes the following positions:
a. The nurse takes the position as close as possible to the client at the side of the
bed straight on the client's abdomen in the direction of the lateral position (eg,
to tilt the right, the nurse is on the right side of the client
b. Upright waist position to protect joints and ligaments.
c. Spread the distance of both feet to maintain stability when pulling the client's
body
d. Knees and hips flexed / flexed
4. Then put the right hand straight beside the client's body to prevent the client from rolling
when pulled into the lateral position (as a buffer)
5. Then put the client's left hand across the chest and the left leg crosses over the right leg
in order to give strength when in the thrust
6. Then tighten the gluteus and abdominal muscles and the flex legs prepare to pull the
body of the client make sure to use the longest and strongest muscles in the legs in order
to prevent trauma and maintain stability.
7. Place the nurse's right hand on the groin of the client and the left hand is placed on the
client's shoulder
8. Then pull the client body toward the nurse by:
a. Strengthen the spinal muscles and slide the nurse's weight to the buttocks and
legs
b. Add more leg flexion and the nurse's pelfis in the lower back to maintain balance
and stability
c. Make sure the client's position remains comfortable and still be able to breathe
9. Then set the client's position by rewarding the pillow on the important part:
a. The client's body is on the side and both arms are at the front of the body with
a flexion position
b. Give a pillow to the head so as to avoid abduction and adduction in the neck
joint
c. Then apply a pillow as a wedge between both arms and chest to prevent chest
muscle fatigue and lateral flexion and to prevent / limit internal rotational and
abduction functions on the shoulders and upper arms
10. Give pillow backs on the back of the client body when needed to provide the right
position
11. Trim the client and clear the unused tool
12. Document the actions that have been done
