Drug diversion is a serious issue that’s more prevalent
than most care to admit.

Allina Health Consolidates

20 Sites on a Single Server
Unity Platform Facilitates Efficiency and Continuity
In 2014, Omnicell completed an installation of automated
dispensing cabinets at 11 hospital sites for Allina Health, a large
not-for-profit health care system based in Minnesota. In addition
to the hospitals, a cabinet was installed in each of nine ambulance
stations. Across Allina Health, the installation involved about 325
cabinets and training of 8,700 users. For all these numbers, only
one server was required.
Through the Omnicell Unity enterprise platform and expanded
capabilities of the OmniCenter ® server, Allina Health has a unified
database and centralized server operating across its multiple sites.
The immediate benefits related to the implementation included:
Reduced infrastructure costs:
There was no need to buy separate servers for each site. Money
was saved on interfaces too. For example, Allina Health didn’t have
to buy 11 sets of interfaces for their Epic EHR.
Streamlined testing process:
Instead of taking 3 to 4 weeks to test servers at each site, testing
for all 20 Allina Health sites was completed in about 3 weeks.

needed to assign unique ID numbers to the various volumes a
liquid medication comes in; previously the same ID number was
used for different volumes of the same medication.
Pharmacy also has to come to agreement on policies and
procedures that affect how cabinets are used (e.g. establishing
override policies and user templates). This takes planning and
communication among multiple departments far in advance of
planned implementations.
In a few months, Allina Health will be installing the Anesthesia
Workstation in its operating rooms, to improve management of
anesthesia medications. With the Unity platform in place, the
installation process should be a smooth one.

Easier to Add New Sites
The Omnicell medication dispensing system will soon be installed
in an additional Allina Health site. The new cabinets can be
connected to the existing server.
More Efficient Software Upgrades
Server work involved in upgrading to a new version of Omnicell
software might have stretched out over a couple months for 20
sites, but instead was recently accomplished in a single evening.
More Consistent User Experience
Protocols and procedures pertaining to cabinet use have been
standardized (some tailoring was done to meet specific site needs),
so a clinician who works at multiple Allina Health facilities will find
it easier to use the cabinets at each site. The same user ID can
be used at each site.
“Our pharmacy team is experienced managing operations across
our numerous and varied sites,” said Ann Byre, PharmD, the
Director of Pharmacy Services at Allina Health. “With good
teamwork between Pharmacy, Nursing, IS and Omnicell, we were
able to establish best practices and achieve the intended benefits
of the Unity platform.”

Omnicell Technology Promotes Nurse Efficiency
As a renowned teaching center and Magnet hospital, Hackensack
University Medical Center (UMC) is continually working to improve
its medication distribution processes. When replacing an older
automated medication dispensing system with the Omnicell Unity
platform, pharmacy leadership embarked on a study to determine if
the new technology—and related workflow enhancements—would
improve efficiency of the medication administration process.
Specifically, Hackensack UMC sought to measure the difference in
time to first dose for piperacillin-tazobactam, a time-critical IV
antibiotic, as well as time to administration for scheduled doses of
all IV antibiotics—compared with their previous system. Missing
doses and nurse satisfaction with the new dispensing system were
also measured.
Hackensack UMC was already meeting the CMS guideline for
administering time-critical medications within a 30-minute time
frame, but the researchers anticipated shaving 10% off their

expectations:
• 40% reduction in time to first dose for piperacillin-tazobactum
• 13.3% reduction in scheduled time to administration time for
all IV antibiotics
Fewer Missing Doses
The study also looked at the prevalence of missing doses, as
indicated by in-basket messages from nursing to pharmacy, which
are captured in the electronic medical record. Results showed a
17.6% reduction of these in-basket messages.
How Were the Improvements Achieved?
Under the previous automated dispensing system, IV antibiotics
were prepared and labeled in the pharmacy, and then delivered to
the unit. As many clinicians can attest, this process often results
in delays, errors, and missing doses.
The Omnicell system enabled a completely new workflow, as
indicated in the diagram below. The IV antibiotics can now be
stocked in the cabinets due to the integrated Medication Label
Printer. As nurses remove IV bags from the cabinet, they print
a patient-specific label to apply to the medication.

Other aspects of the Omnicell system also contributed to nurse
efficiency:
• Patient-specific medications are automatically managed through
the cabinet, giving nurses a single point of access for acquiring
medications.
• A system for remotely queuing medications shortens time spent
at the cabinet.
• A single-dose dispensing module virtually eliminates the need to
conduct countbacks of controlled substances.
In addition, Hackensack UMC also deployed interoperability
between the Omnicell and Epic systems, which allows nurses to
manage medications within the electronic health record (EHR).
Nurse Satisfaction
As part of the study, 122 nurses were surveyed at two different
time points about their experiences with the Omnicell medication
dispensing system compared to the previous system deployed at
Hackensack UMC. Results showed statistically significant
improvement in nurse satisfaction regarding many workflow
factors, including efficiency of medication administration related
to the new system.

“The administration time for antibiotics is critically important for
preventing infection and promptly treating infection,” said Nilesh
Desai, BS, RPh, MBA, Director of Pharmacy for Hackensack UMC.
“The Omnicell medication dispensing system enabled us to refine
our processes and get medications to patients faster—and that
allows us to improve patient care.”
The results of the study were presented by Hackensack UMC at
the New Jersey Society of Health-System Pharmacists annual
meeting on April 24, 2015.

The second compliance date is fast approaching for manufacturers
to apply a unique device identifier (UDI) to both Class III devices
and Class II and I implants that are life-supporting or life-sustaining.
The deadline for Class III devices was September 2014, but
manufacturers were allowed to request a one-year extension.
That extension end and deadline for use of UDI on Class II
implants is September 2015.
