Cell-based phenotypic profiling and image based high-content screening are used to gain insight into the mode of action and potential cellular targets of plants
historically used to determine anti-cancer activity of Saudi Arabian plants Juniperus phoenicea (Arar), Anastatica hierochuntica (Kaff Maryam), and Citrullus colocynthis
(Hanzal). The cytological profiles of fractions taken from the plants were compared with a set of reference compounds with known modes of action. Cluster analyses of
the cytological profiles were performed, which revealed detailed information on the modes of action of the tested compounds as potential topoisomerase inhibitors.
Cytological profiles showed that some of these compounds inhibited cell proliferation causing cell cycle disruption.

Objective The objective of the present study was to determine whether a preparation made from Emblica officinalis (Nelli/Amla) and Tinospora cordifolia (Rasakinda/
Guduchi) has a prognostic effect in the treatment of oral cancer. Method This study was a randomized double blind prospective clinical trial conducted at Maxillofacial
Unit, Teaching Hospital, Karapitiya, Galle during the period of 2005 to 2014. The study group comprised 114 consecutive patients with oral squamous cell carcinoma
(OSCC) who had attended Maxillofacial unit, Teaching Hospital, Karapitiya, Galle. Of this 70 had carcinoma in the Buccal mucosa and 44 in the Tongue. After
conventional treatment of these patients, they were randomly assigned to the Trial group of 51 and Control group of 63. The Trial group was given a herbal treatment
consisting of Emblica officinalis (Nelli) and Tinospora iicordifolia (Rasakinda) in powdered form. The Control was given a placebo (Viva milk powder) instead. Efficacy of
herbal regime to improve the prognosis of oral carcinoma was determined by recurrence free survival and incidence of recurrence. These values in relation to age,
gender and stage of the lesions were compared using SPSS soft ware and chi square statistical method. Results According to the results, both OSCC Buccal mucosa
and Tongue trial groups gave better prognosis. The study managed to demonstrate that local recurrences and gross recurrence of OSCC Buccal mucosa were greatly
reduced in the trial group, with trial group showing statistically significant (p = 0.02) for local recurrence and (p = 0.05) for gross recurrence. Similarly, in the trial group of
OSCC Tongue, there was a remarkable reduction of nodal recurrence and gross recurrences. This was highly statistically significant for nodal recurrence (p = 0.1) and
for gross recurrence (p = 0.1). In the OSCC Tongue trial group there was a greater reduction of nodal recurrences whereas a greater reduction of local recurrence
observed in the OSCC Buccal mucosa trial group. The reduction of recurrences in trial groups of carcinoma of buccal mucosa and carcinoma of tongue led to better
survival rates. Five-year recurrence-free survival of the trial groups of both carcinoma together, was 62.7% compared to 42.3% in the control group (p = 0.031) and
statistically significant. iii Conclusion It is justifiable to conclude that the herbs used in this study, Emblica officinalis (Nelli / Amla) and Tinospora cordifolia (Rasakinda /
Guduchi) preventedcancer recurrence of tongue and buccal mucosa cancer during a 5 year follow up period, which led to an overall improvement in the survival rate.
And in conclusion pharmaceutical or medicinal preparation comprising a combination of two herbs Emblica officinalis (Nelli) and Tinospora cordifolia (Rasakinda) can be
administered together for targeted stimulation of immune system for prophylaxis and treatment of oral cancer.
