Over-the-Rhine Urban Kickball League – Fall 2010

Thank you for your interest in the first season of the Over-the-Rhine Urban Kickball League. You will find all
the information you need to know within this document including the necessary sign-up forms, rules, and
payment instructions.

Date/Time: Tuesdays, Wednesdays, and Thursdays at 6pm - one game per team, per week over the six-
week schedule.

Location: All games will be held at Cutter Park (beside the former School of Creative & Performing Arts)
along Sycamore Street.

Teams: Teams will consist of 10-12 members each and have a 50/50 split of men and women. Game
play will require 8 players on the field at a time. If you sign up individually we will place you on a
team that needs to fill out their roster unless you otherwise specify who the team leader (or
another team player) is for the team you wish to join.

Cost: $5 per player between 9/17/2010 and 9/24/2010; $10 per player after initial 9/24/2010 sign-up
deadline. Please pay cash, or make checks payable to “Randy Simes” and return with sign-up
form.

Games: Games will last approximately 1 hour. Referees, balls, and all necessary equipment will be
provided. Simply come prepared to play with a minimum of 8 players for your scheduled time
(to be posted on Facebook group page and sent via email to team leaders).

Season: The season will last six weeks in total. The first game of week one will take place on Tuesday,
October 5 unless an unexpectedly high response occurs. If that takes place, then the first game
of week one will take place a week later on Tuesday, October 12.

Submission: Please submit sign-up forms, with cash/checks, to Neon’s Unplugged located at 208 E. 12th
Street. Additional forms will be available for pick-up there. Please submit forms/checks in an
enclosed envelope with your name, or team leader’s name, marked on the outside of the
envelope. Each player must also sign and return a signed “Waiver of Liability” (page three).

Team Shirts: Shirts will be provided to every player. Please indicate your preferred size in parenthesis
behind your name on the sign-up form. Ex. “Randy Simes (M)”

 WAIVER OF LIABILITY
THIS IS AN IMPORTANT LEGAL DOCUMENT. READ CAREFULLY BEFORE SIGNING.

In consideration of acceptance of my entry into the OTR Urban Kickball League, I, for myself, my heirs, my
executors, my administrators, my trustees, and any and all successors in interest, fully and forever waive and
release any and all rights and claims for any injuries and damages, including but not limited to demands or
actions for negligence, premises liability, emotional injury, intentional conduct, resulting in unintended injury or
damage tort claims, any other actions or demands of whatsoever nature, to the fullest extent permitted by law,
I may have against (1) OTR Urban Kickball League; (2) Cutter Playground; (3) City of Cincinnati; (4)
UrbanCincy; (5) all sponsors of the OTR Urban Kickball League; and (6) all employees, principals, directors,
shareholders, agents, members, managers, affiliates, volunteers, officials, and representatives acting for or on
behalf of any of these entities.

I acknowledge that I am aware of the inherent risks involved in the event, and I voluntarily assume these risks.
I attest and verify that I am physically fit and I have sufficiently trained for the competition in OTR Urban
Kickball League, and that a medical doctor has verified my physical condition.

As a condition of my participation in this Event, I hereby grant OTR Urban Kickball League a limited license to
use my name, likeness, image, voice, video, athletic performance, biographical and other information
(collectively, “Likeness”), in any format whatsoever, and to distribute, broadcast and exhibit these without
charge, restriction or liability, but only for the purposes of advertising or promoting the Event, UrbanCincy, or
the sport of Athletics. The foregoing grant, however, does not constitute consent to use my Likeness in an
endorsement of any product or service without my specific written consent.

The OTR Urban Kickball League reserves the right to reject my entry and further reserves the right to change
the details of the event without prior notice. I understand that my entry fee is non-refundable.
