OUTPATIENT PHYSICAL THERAPY WORKPLACE RENOVATION

1. Project Purpose
Renovate desk and area to create a more functional work station for physical therapists and staff.

2. Deliverable
Construction of new desk and work station.

3. Project Scope
The scope includes the planning for and construction of the work space, contacting vendors, and budgeting of the
project.

4. SMART Objectives
 The work station will be ready within 21 days.
 Not to exceed budget of $10,000
 Provide more storage space for therapy literature and resources.

5. Stakeholders and Expectations

Stakeholders include: physical therapists, physical therapy aides, management team, and physical therapy patients.

 We expect that the creation of this work space will increase functionality of the area for physical therapy
aides and physical therapists.
 There will be an increase in storage space which will allow for higher productivity.

6. Assumptions
 Construction process will not interfere with patient flow.
 $10,000 is final budget amount
 Management team will support the project

7. Constraints
 Budget (cannot exceed $10,000)
 Schedule (cannot interfere with patient care and must be completed within 30 days)
 Construction needs to be in compliance with all safety regulations

 8. Risks
 Actual construction costs go over estimated cost
 Construction can only be done during work day hours
 Furniture delivery gets delayed.

9. Communication Plan
 Communicate weekly with management team on status of project, issues, and milestones
 Communicate with construction services and vendors to coordinate timelines as needed

10. Sponsor
Physical therapy management team

11. Members and Their Expectations

 Construction Services – Provide detailed quote, deliver on construction needs, installation of new furniture
 Vendor – Provide detailed quote, prompt shipment of selected furniture
 Physical therapy management team – Provide assistance with approving processes quickly
