The Samahan ng mga Detinado ng Tarlac Provincial Jail (SDTPJ) is

requesting for the approval of the following activity for this month’s Family Day
Celebration on January 27, 2018 at 7:00 o’clock in the evening at the Bulwagan
Stage. We will be staging a pageant dubbed as Ms. Gay Barako showcasing talents
and beauty of male inmates cross dressed as females.

SUBJECT : Request for Medical Officers for Medical Mission

DATE : November 24, 2016

1. Reference: Weekly medical check-up of inmates by Dr Raymund Talon of

Talon General Hospital on November 14 and 21, 2016.

2. Some inmates are suffering from illnesses such as diabetes, hypertension,

bleeding disorder, cough and colds, and uterine problems.

3. In view of the foregoing, may we request for medical doctors particularly

OB-Gynecologist and internal medicine doctors preferably on Mondays,
Wednesdays, and Fridays as scheduled non-visiting days at Tarlac Provincial Jail.
