Greetings!

In connection with the Inmate Welfare Development Program and Healthcare

Program of TPJ, the Samahan ng mga Detinado ng Tarlac Provincial Jail (SDTPJ) is
requesting for the approval of the volleyball program for female inmates. This activity is
requested to be conducted at the Bulwagan volleyball court every Tuesdays, Thursdays,
Fridays, and Saturdays at 4:00 PM to 6:00 PM.

SUBJECT : Request for Medical Officers for Medical Mission

DATE : November 24, 2016

1. Reference: Weekly medical check-up of inmates by Dr Raymund Talon of

Talon General Hospital on November 14 and 21, 2016.

2. Some inmates are suffering from illnesses such as diabetes, hypertension,

bleeding disorder, cough and colds, and uterine problems.

3. In view of the foregoing, may we request for medical doctors particularly OB-
Gynecologist and internal medicine doctors preferably on Mondays, Wednesdays, and
Fridays as scheduled non-visiting days at Tarlac Provincial Jail.
