NORMAL REGISTRATION
Normal birth registration application means every birth occurring in Malaysia should be registered within 14
days from the date of birth of the child.
APPLICATION PROCEDURES
Birth registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting
the completed Form JPN.LM01 and attaching supporting documents.

LATE REGISTRATION
Late birth registration application means every birth occurring in Malaysia which is registered after 42 days
from the date of birth of the child.
APPLICATION PROCEDURES
Late birth registration application can be made at any NRD Office throughout Peninsular Malaysia by
submitting the completed Form JPN.LM01 and attaching the supporting documents. Each application made
will be accepted after the result of the search is obtained by using form JPN.LM14 (except birth which
occurs after 2003).

DELAYED REGISTRATION
Delayed birth registration application means registration that is made within 15 to 42 days from the date of
birth of the child and the birth occurs in Malaysia.
APPLICATION PROCEDURES
Birth registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting
the completed Form JPN.LM01 and attaching the supporting documents.

Each birth which occurs while on pilgrimage should be reported to the registrar in Hedjaz within 40 days
from the date of birth or reported to the NRD Headquarters counter within 6 months from the date of birth.
However, approval will only be given by the Registrar-General at NRD Headquarters.

Hedjaz birth registration application can be made at any NRD Office throughout Peninsular Malaysia by
submitting the completed Form JPN.LM01 and attaching the supporting documents.

RE-REGISTRATION (MISSING & DESTROYED)
Birth re-registration application is registration that is made because the register or the record of the birth
cannot be traced, probably missing or destroyed, by submitting proof that the birth has been registered.

Applicants need to complete form JPN.LM01 and attach the supporting documents. Birth registration can be
made any NRD Office throughout Peninsular Malaysia.

An application for the re-registration of birth of a legitimated person is a re-registration of the birth of an
illegitimate non-Muslim child who has been registered and then legitimated.

made any NRD Office throughout Peninsular Malaysia.

ALTERING NAME OF CHILD
Application to alter the name of a child which has been registered can only be made within 1 year from the
date of birth of the child. However, the name of a child that has been registered CANNOT BE ALTERED if
the child has attained the age of 1 year or more from the date of submission of application.

Application to correct minor error which only involves spelling error which if corrected does not alter the fact
in the Birth Certificate.
