Brief Description of the Incident/Accident:

As per the investigation mechanical team (S/C Manish indrajith) Workers are
remove GI shed which was provided for material storage purpose. CO2 type
Fire Extinguisher (9kg capacity) & wooden box are stored in the shed. During
the work the worker was used Arc welding for cutting of ms pipe. due to
negligence of the worker welding spark fall on the hessian cloth (fire
extinguisher packing cover )and catch fire ,immediately extinguish the flame at
starting stage . Fortunately no any injury in this incident 4 no’s of Fire
Extinguisher valve damaged fully CO2 gas discharged.
