CONSUMER AFFIDAVIT OF ELIGIBILITY & LIABILITY/PUBLICITY RELEASE

In consideration of the participation in the "DEADPOOL CORE DEADPOOL IT YOURSELF" PROMOTION ("Promotion"), I agree as
follows:

1. I am a United States citizen, am 18 years of age or older; am not an employee, officer, director, agent or representative
of Twentieth Century Fox Film Corporation, their affiliates, subsidiaries, advertising and promotion agencies (collectively, “Promotion
Entities”), and the independent judging organization and immediate families of each.

2. I release Promotion Entities, and their employees, agents, officers, directors, principals, partners, subsidiaries,
successors and assignees from all injuries, claims, losses, expenses, costs or liabilities of any nature that I may have against Promotion
Entities, and their employees, agents, officers, directors, principals, partners, subsidiaries, successors and assignees, whether known or
unknown, that are based on or arise out of the conduct of the Promotion.

3. I understand and agree that this general release applies to all claims I may have that are related to the Promotion,
whether the claim is now known or unknown. Having been so apprised, I nevertheless assume all risks for any claim I might have, arising
before or after this date, whether known or unknown to me, relating to the Promotion.

4. I waive any right I may have to dispute the scope of this general release on the grounds that it is a general release and
a release of unknown claims.

5. I hereby grant Promotion Entities, and their agents, and others working for it or on its behalf and their respective
licensees, successors and assigns (hereinafter "Licensed Parties") the absolute right and permission to use, publish, broadcast and
copyright my art submission (“Submission”), voice recording, name, picture and likeness or any material based upon or derived therefrom,
or to refrain from so doing, in any manner or media whatsoever for purposes of advertising or trade in promoting and publicizing its
business, products, services and the Promotion.

6. I agree that Licensed Parties shall be the exclusive licensee of any Submission, and the exclusive licensee of any
voice recording, picture or likeness of me or anything derived therefrom. If I should receive any print, negative or other copy, I shall not
authorize its use by anyone else.

7. I shall have no right of approval, no claim to additional compensation and no claim (including without limitation, claims
based upon invasion of privacy, defamation or right of publicity) arising out of any use, alteration, distortion or illusionary effect or use in
any composite form.

8. I agree that this release does not in any way conflict with any existing commitment on my part.

9. I represent to Promotion Entities that:

a) I have read, fully informed myself of and fully understand all the terms and contents of this general release
and that no promise or representation of any kind has been made by Promotion Entities, and

b) I fully understand that this is a full and complete release of any claims that I might ever have against
Promotion Entities with respect to the Promotion.
