Hialalco shall include in the Risk Assessment consider the following general hazards and
risks associated with the use of lifting Equipment and lifting accessories in Section 3.4.3(b)
5 Risk Assessment of OSHAD -SF CoP 34.0 Safe use of Lifting Equipment and Lifting
accessories.
Hilalco's Lifting Permit format to be changes use /follow the approved Lifting Permit
6 Format that has been provided.

Note that Specific Lifting Plan and Risk Assessment had to be provided for Specific
7 kind/type of site activities, and to be forwarded to KBR/Hyder for review prior for
approval.
