SECURITY DEPOSIT
Large Groups (over six adults) may be required to send a security deposit. Please call 262-290-5040 to determine amount of security
deposit. All security deposits will be refunded if the property is returned to the same condition it was found at check-in. It is the renter’s
responsibility to inform Cabin Wisconsin of any damage that exists at the rental property prior to renter’s check-in on the date of check-in.

CANCELLATION POLICY
All reservations are subject to a 21 day cancellation policy. You must cancel at least 3 weeks prior to the start date of your reservation
to receive a 100% refund. If a booking is canceled less than 21 days prior to the reservation start date, the entire amount of reservation
is non-refundable. ALL CANCELLATIONS MUST BE IN WRITING at sales@cabinwisconsin.com or Lake Escapes, LLC dba Cabin
Wisconsin, 2940 S. 127th St., New Berlin, WI 53151.

PETS
We do have cabins that are pet-friendly! If you’re interested in including your pet please be sure and fill out the Pet Addendum Form and
mail it in along with your Rental Agreement Form.

LOSSES AND DAMAGES

Any and all damages to the home or negligence resulting in damage to the home or contents will result in costs to repair being deducted
from the security deposit or billed to guests if said damage or loss of contents is greater than security deposit. Guest(s) shall be solely
responsible for any property damage, accident/injury to any person or loss sustained by any person, including loss of money, jewelry,
and other items of personal property, arising out of or in any way related to Guest(s) use of premises or the items of personal property
provided by Cabin Wisconsin at Guest(s) request. Guest(s) shall inspect and be familiar with property use and application of such prior
to using them.

Guest(s) hereby agrees to INDEMNIFY and hold Lake Escapes, LLC harmless of any and all claims including those of third parties,
arising out of, or in any way related to Guest(s) use of premises or the items of personal property therein. Guest(s) assumes the risk of
injury or other losses relating to any recreational activities while staying at the property. Guest(s) will hold Lake Escapes, LLC and its
Agents, and Owner(s) of Property harmless with respect thereto. LLC, Owner and Agents are not responsible for articles left on premises.
There will be a $10 charge in addition to shipping charges for handling the return of any articles found by housekeeping.

SMOKING
NO SMOKING is permitted inside any of the cabins. Evidence of smoking may result in partial or complete loss of security deposit.

CHECK-OUT PROCEDURE
Guest(s) shall leave premises in neat and undamaged condition. Complete the CHECK-OUT CHECK LIST on the following page before
leaving the premises. Any guests not completing the CHECK-OUT CHECK LIST will be subject to a deduction of $50 from their security
deposit for additional cleaning/maintainence expenses.

KEYS
Each cabin has secured entry with a lock box marked with the Cabin Wisconsin logo. Cabin keys are located inside the lock box. A lock
box combination will be provided to you when your reservation has been processed. Please place cabin keys inside the lock box at the
end of your stay.

RETURNED CHECKS
A $25 service charge will be incurred for any returned check.

NO EXTRA GUEST/ NO HOUSE PARTIES

Rental Guest understands that we will accept families, couples and responsible adults over the age of 21 ONLY. Guests testifiy that he/she
is of legal age to enter into a contract in the State of Wisconsin. Guest(s) agree that the premises shall not be occupied by more than
the number of people stated on the reservation. If the unit is occupied by more than the number of people stated, it will result in loss of
security deposit. Occupancy in use of premises shall not be such as to disturb or offend neighbors. The use of firearms, or fireworks is
strictly prohibited.

VIOLATING AGREEMENT
If guest(s) violates any of the conditions of this Agreement, Agent may terminate this Agreement and enter Premises. Upon notice of
termination of this Agreement, Guest shall vacate the Premises immediately. Either by signing this or acknowledging through the online
reservation system, I have read and fully agree to all the above policies.
PET ADDENDUM (If applicable - please complete and sign)

GUEST AGREES:
1. To first seek permission from Lake Excapes, LLC dba Cabin Wisconsin before bringing dog or other pet
to premise. The size and breed of dog must be acceptable by Lake Escapes, LLC dba Cabin Wisconsin.

2. Pet will not damage premises. If by chance damages are caused, the cost of the damages will be
deducted from security deposit.

3. Pet will not annoy, endanger, or inconvenience neighbors. Pet owner assumes all liability to actions of the
animal.

4. Pet will not be left unattended inside or outside cabin.

Row boat TURNED over and tied to dock (if included)

ALL BEACH TRASH THROWN AWAY (if lakefront property)
