Teacher Goals (in measurable terms):


1. By June 2018, Mr. Rigs will demonstrate growth in his ability to plan a coherent lesson using the AMS 5-E Lesson Plan Template, in
which all activities support the instructional outcome of the lesson, as evident by scoring effective and evident in the area of Domain 1
on formal and informal observations.
2. By June 2018, Mr. Rigs will demonstrate growth in his ability to establish a culture for learning in his classroom by setting and
enforcing behavioral and academic expectations within the classroom, as evident by score effective and evident in the area of Domain 2
on formal and informal observations.
3. By June 2018, Mr. Rigs will demonstrate growth in his ability to engage students in learning by implementing lessons with a clearly
defined structure, activities and assignments that are appropriate to students and cognitively engaging, and development of
productive instructional groups, as evident by scoring effective and evident in the area of Domain 3 on formal and informal
observations.

12/14/17 Meeting Participants: Mr. Rigs (Teacher); (DC); Perry Warren (AP)
 Meeting participants discussed the lesson planning process and lesson plan submission. Mr. Rigs will present
Teacher Initials:________ two written lesson plans for lessons he plans on teaching the following week. Dates were set for Mr. Rigs to
submit the first two plans to the department chair and Mr. Warren (Lesson 1: 12/18/17; Lesson 2: 12/20/17).

Lesson Plan Template.
 Mr. Rigs expressed his current behavior management plan is producing beneficial results. He will detail this
plan in writing and submit it to the department chair and Mr. Warren on 12/22/17. Components of his plan
will include class rules, the sequence of consequences, and a parent contact log.
 The science department chair and Mr. Rigs will continue to work together during collaborative planning time
to plan weekly lessons.
 Mr. Rigs, the department chair, and Mr. Warren will meet on biweekly basis to reflect on Mr. Arthur’s
planning, instructional, and behavior management progress and provide Mr. Arthur with feedback designed to
support the goals of the performance plan.

 Meeting participants discussed the lesson planning process. Mr. Rigs expressed that developing the lesson

lesson logistics, resulting in more fluid transitions between providing students directs and students starting
their work. Mr. Rigs is encouraged to continue to use to the feedback provided on his lesson plans and
develop detailed plans that provide explicit outlines of both content and pedagogy.
 The team discussed Mr. Rigs’ progress on his Teacher Development Plan and Student Learning Outcomes
(SLO). The SLO completion date is February 28th. Mr. Rigs stated that his SLOs are on track and that the pre
and mid assessments have been administered.
 The team reviewed Mr. Rigs behavior management plan. Mr. Rigs has been implementing his plan with fidelity
and making the necessary parent contacts.
 Mr. Rigs was informed of the mid-year evaluation process and provided the Danielson Rubric to use as a
resource for his self-assessment.

 Participants engaged in conversation about Mr. Rigs performance, within each of the four domains, since the

 Mr. Rigs is showing growth both in the planning process and classroom instruction. He is encouraged to
continue to enhance the level of detail, specifically in pedagogical rational, as he develops lesson plans. Mr.
Rigs will continue to submit two lesson plans weekly and will be provided feedback.

Following Mr. Rigs formal observation some of his weekly plans failed to be submitted. During this meeting
