Reno, NY 89505 Certified Mail: 71067808063000678117 RE: Dam Safety Inspection of the WEST WASH DAM, 1-047, NW00078 Dear Steve Gamilis: (On May 9, 2018, Melissa Marr and Amanda Brownlee, P.E, of the State Engineers office, inspected the WEST WASH DAM. The purpose of the inspection was to determine the ‘condition ofthe strcture with respect to dam safety. ‘The WEST WASH DAM is classified asa High hazard dam due to downstream development, ‘As this dam is classified asa significant or high hazard dam, an Emergency Action Plan (EAP) ‘must be developed, exercised and updated regulary, The EAP for this dam was last revised February 1, 2006, An EAP should be reviewed yesrly ata minimum and updates provided tall the plan holders listed within the EAP. ‘The overall condition ofthe dam is FAIR. Please note that his physical inspection may not identify all potential risks with this dam and the inspection report isnot intended toe an. evaluation ofthe proper operation and management of the dam, The dam owner is encouraged ‘o seck qualifies resourees to ensure proper maintenance; regulatory compliance and dem safety ‘management are carried out. The owner is ultimately responsible forthe safe operation and performance oftheir dam. A dam owner should also be familiar with the legel ligation to ‘maintain and operate the dam ina safe condition. Based onthe engineer's observations, the following corrective actions should be taken: |. PUBLIC SAFETY/SECURITY - Update EAP: The curently filed Emergency Action Plan (EAP) has not been updated recently. Review the notification flow chart and update the EAP as needed and submit the revision to all interested parties. An update on the EAP thet was being developed last year must be submitted to this office within 30 days of the date of this let, Long Term (2 Years) 1. CREST - Loss Of Crest Width: Placement of new materials along the rest should be considered to restore designed erest width, - Photos Atached. 2. AUXILIARY SPILLWAY - Encroachment: Unauthorized eneroachment wes observed. Further analysis ofthe impact of encroachment on the dam is recommended. - Photos Attached, 3. ALL DAM - Inappropriate Uses: Possible inappropriate uses ofthe dam were observed, ‘Consider modifying curent practices and installing access prevention measures. - Photos Attached, 4, ALL DAM - Rodent Activity: Rodent setvity should be monitored and any holes iscovered should be backilled and properly compacted. A rodent contol program should be instituted 5. ALL DAM. Vegetation: Remove vegetation within 25 fet ofthe structure and disposed of offsite. - Photos Attached. ‘The above items are considered maintenance issues and do not require a new approval of plans and specifications (permit) to accomplish.
