RE: Dam Safety Inspection of the WHEELER RESERVOIR DAW, (NV0O072)
Dear Ms. King:
(On August 29,2017, Christopher Thorson, P.E., Melissa Marr and Emily Maher ofthe State
Engineers office, inspected WHEELER RESERVOIR DAM. The aurpose of the inspection was
{o determine the condition ofthe structure with respect to dam safety. The WHEELER
RESERVOIR DAM is classified asa High hazard dam due to dowastream development,
Based on discussions with Mr. Rick Schwarze, you are inthe process of completing an update to
the Emergency Acton Plan, This ffice commends you on keeping this document up to dae,
Please ensure that al the plan holders recieve the updated pages or document.
Also, there isa potential plan to modify the downstream slope of he northern dam. Prior to any
construction ofthis face, a Notice of Construction must be submited and approved by this,
office. Depending on the work planned, this might entail an application tobe submited to this
office. When you ae further along in the planning process, please contact this office to cnamme

‘The overall condition of the dam is EAIR. Based on the engineers cbservations, the following
corrective actions should be taken:
Short Term (1 Year)
|. CREST - Other: Continue to rebuild the french drain onthe southem dam. You must have
plan that note the tein location and details of fench drain system. This wil be helpful i
the future if there isan issue with this system. -Photos Attache
2. CREST - Vegetation: Minor vegetation observed along the crest should be removed and
deposited offsite. - Photos Attached.3. PRINCIPAL SPILLWAY - Downstream Channel Conditions: The outlet channel will need
tobe cleared ofall vegetation to prevent a backwater condition and allow the fll ow of
water tobe discharged from the outlet conduit. - Photos Attached.
4. ALL DAM - Rodent Activity: Significant rodent sctivty and burrows were observed, You
‘must properly backfill and compact any holes o prevent further propagation. A rodent
control program should be inaiuted
Long Term (2 Years)
1. UPSTREAM FACE - Vegetation: Deep rooted vegetation and trees on the embankmert face
shouldbe removed andthe slash material deposited offsite
‘The above items are considered maintenance issues and do not require @ new approval of plans
and specifications (permit) t accomplish. Please be aware that future modifications may equi
submittl of plans and specifications for review and approval (permit),
