RE: Dam Safety Inspection of the HIGHLAND RESERVOIR, J-464 (NV00067)

Dear Bill Hauck:

On August 29, 2017, of the State Engineer's office, inspected the HIGHLAND RESERVOIR.
The purpose of the inspection was to determine the condition of the structure with respect to
dam safety. The HIGHLAND RESERVOIR is classified as a High hazard dam due to
downstream development.

The overall condition of the dam is FAIR. Based on the engineer's observations, the following
corrective actions should be taken:
Short Term (1 Year)
1. ALL DAM - Rodent Activity: Minor rodent activity was observed. You must backfill any
holes and prevent further propagation. A rodent control program should be instituted.

2. DOWNSTREAM FACE - Erosion Gullies: Erosion gullies should be filled with suitable
embankment fill and compacted properly. - Photos Attached.

The above items are considered maintenance issues and do not require a new approval of plans
and specifications (permit) to accomplish. Please be aware that future modifications may require
submittal of plans and specifications for review and approval (permit).
