LETTER OF AUTHORIZATION

To whom it may concern:

This is to authorize my mother, RHODA L. SANTOS, to receive my RCBC

Bankard Credit Card on my behalf. Due to the fact that I will not be at home when
the said credit card is going to be delivered, I therefore issued this letter of
authorization as compliance to the requirement of RCBC.

With her are photocopies of my two valid ID’s, namely: 1) Integrated Bar of
the Philippines Card and 2) Driver’s License.
