APPLICATION
Comes now the applicant through counsel, and to this Honorable Court,

most respectfully applies to have the parcel of land hereinafter described brought

under the operation of the Property Registration Decree and to have title thereto

confirmed in its name, and declare:

1. That the applicant is a one hundred percent Filipino owned

corporation, duly organized and existing under and by virtue of the Laws of the

Republic of the Philippines, with business address at Suite 806 Globe Telecom

Plaza, Pioneer St., Mandaluyong City, represented herein by MR. ED MAR

VINCENT L. BONOAN;

2. That the applicant is the owner in fee simple of a certain parcel of land

situated at Brgy. 2, San Baltazar, San Nicolas, Ilocos Norte, more particularly

described as follows:

A parcel of land (Lot No. 2540 of the Cadastral Survey


of San Nicolas). Bounded on the North by Lot Nos. 2539 and
2536; on the East by Lot No. 2535; on the South by Lot No.
2515; and on the West by Lot No. 2541, containing an area of
ONE THOUSAND ONE HUNDRED TWENTY FIVE (1,125)
SQUARE METERS;
having acquired the same by purchase from Victoria U. Abuluyan, copy of which

is hereto attached and made an integral part hereof;

3. That the said parcel of land was assessed for taxation purposes for the

current year at P36,000.00, machine copy of the Declaration of Real Property is

hereto attached and made an integral part hereof;

4. That the applicant to determine the exact metes and bounds of the

aforesaid property had engaged the services of a private land surveyor who re-

surveyed the parcel of land and thereafter worked out for the approval of the

Advance Plan by the Director of Lands;

5. That the accurate description of the aforesaid parcel of land is shown

in the approved plan and the approved technical description issued by the

Regional Executive Director, Land Management Services, Region No. 1, DENR,

San Fernando City. The approved plan (Tracing Cloth Plan) and the

corresponding technical description are hereto attached and made an integral

part hereof;

6. That the aforesaid parcel of land had been surveyed for the herein

applicant in accordance with the existing laws as shown by the Certification in

lieu of the Geodetic Engineer’s Certificates for titling purposes hereto attached

and made an integral parts hereof;

7. That to the best of applicant’s knowledge and belief, there is no

mortgage or encumbrance of any kind whatsoever affecting said parcel of land,

nor is there any person having interest therein, legal or equitable, in possession,

remainder, reversion or expectancy;

8. That the parcel of land being applied for is within the Alienable and

Disposable Land of the public domain;

9. That the applicant has been in open, actual, continuous, uninterrupted

possession of said parcel of land in the concept of an owner and by way of

tacking of possession herein applicant has been in open, public adverse,

continuous and uninterrupted possession in the concept of an owner of the

subject property for more than fifty (50) years;

10. That the records of the Land Management Section, Department of

Environment and Natural Resources show that no patent has been issued, or

that an application for the issuance of patent is pending before the said office,

and that the said parcel of land is not reserved for military or public use;

11. That the said parcel of land is occupied by no other person except the

herein applicant. Herein applicant has never been disturbed in its possession by

any one since the time of its acquisition up to the present;

12. That the names in full and addresses, as far as known to the

applicant of the owner of the property adjoining the property herein applied for

is the following:

13. That the following documents are attached hereto and made as an

integral part of this application:

Annex “B” Deed of Absolute Sale dated October 28, 2005

Annex “C” Tax Declaration No. 08-002-00045

Annex “D” Advance Plan of Lot No. 2540

WHEREFORE, in the light of all the foregoing, it is respectfully prayed of

this Honorable Court that, after due notice, publication and hearing, judgment be

rendered:

 Ordering that the above-described parcel of land be brought


under the operation of the Property Registration Decree;

 Ordering the Land Registration Authority the issuance of a


decree and the corresponding original certificate of title to
the above-named applicant;

 Ordering the Registry of Deeds for the Province of Ilocos


Norte, District 1, Laoag City the registration and
confirmation of title over the aforesaid parcel of land in the
name of the applicant;

 Applicant further prays for such other relief and remedies


deemed equitable in the premises.

I, ED MAR VINCENT L. BONOAN, Filipino of legal age, under oath,


depose and state that:

1. I am the authorized representative of the applicant in the above-


captioned case;

2. I caused the preparation of the foregoing application;

3. I have read the contents thereof and found them to be true and correct
of my own knowledge and based on authentic records;

4. I have not filed any similar action or proceeding involving the same or
related issue in the Supreme Court, Court of Appeals, or any Divisions thereof, or
any other tribunal or agency, and to the best of my knowledge, no such action or
proceeding is pending in the Supreme Court, Court of Appeals or any tribunal or
agency. If I should thereafter learn that a similar application or proceeding
involving the same issue has been filed or is pending in the Supreme Court, the
Court of Appeals, or different Divisions thereof, or to any other tribunal or
agency, I undertake to report such fact within five (5) days from notice thereof
to the Honorable Court.
