I. Instructors

1. Hytham Osama El-Nahhas, Corporate HSE Advisor, BP

Hytham was graduated from Faculty of Science - Ain Shams University with a Bachelor of Science,
Chemistry Major. He has 9 years of experience in the HSE Field. He worked for 6 years in BP JV company
(GUPCO - Exploration & Production Company) as a Safety Eng. in the Gulf of Suez (GOS) Field - Onshore /
Offshore and 3 years in BP Main Office in Maadi as a HSE Advisor. Hytham's main areas of experience
include building the HSE Management Systems; implement the Control of Work & Driving standards,
and Behavior change programs. Also, he participate in major / minor incidents investigations as a root
cause analysis specialist and Safety Management audits.

2. Khaled Nabil Ahmed, Corporate HSE Advisor, BP

Khaled was graduated from Faculty of Science - Cairo University with a Bachelor of Science, Chemistry
Major he had in 2001 , an Environmental Engineering Diploma, and he is now is about to finish his MBA
degree, He has 11 years of experience in the HSE Field as follows:

1. He worked for 6 years in IEOC JV company (Agiba - Exploration & Production Company) as a Safety
specialist in the Western Desert & Gulf of Suez , onshore & offshore Fields.
2. After that he worked for 3 years in United Gas Derivatives Company (UGDC), which is a major NGL
plant as a HSE technical Assistance , in Port Said & Damietta, his worked in the NGL plant during
the construction, commission , start up & Operations phases.
3. After that he worked for 3 years in BP Main Office in Maadi as a HSE Advisor. Khaled’s main areas
of experience include building the HSE Management Systems; implement the Control of Work &
Driving standards, and Behaviour change programs. Also, he participate in major / minor incidents
investigations as a root cause analysis specialist and Safety Management audits.

Day 1: Task Risk Assessment

Task Risk Assessment is a method for systematically examining a job to identify

Day 2: Control of Work (COW)

Description This training provides a formal and consistent approach to managing risk for
employees and contractors - when carrying out work activities on behalf of the
company. The Standard comprises of 12 elements which define the boundaries
within which local control of work processes must exist. It provides the
framework which defines the minimum mandatory requirements for any control
of work process.

Learning  To provide assurance that everyone involved with the CoW process has the

Day 3: Incident investigation & root cause analysis (Basic Level)

Description This training is intended for employees involved in Incidents Investigations and
Root Cause Analysis.

Learning  To Know what your responsibilities are


Objectives  To be able to determine what information is to be gathered during the
investigation
 To be able to effectively interview persons with pertinent knowledge
 To be able to use the 5 Whys and/or Comprehensive List of Causes (CLC) to
determine root or system cause's)
 To be able to develop appropriate conclusions regarding cause's) and write
effective proposals for corrective action
 Explain the reason of why we investigating all incidents/ Accidents and near
misses.
 Describing ways to improve the knowledge of how to carry out an investigation
 Presenting Basic steps to understands the root causes
 How to use the CLC chart to identify the possible immediate causes and
possible system causes
 To assess safety barriers in order to create the required corrective actions.
