MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement is made and entered by between:

The UNIVERSITY OF CALOOCAN CITY represented by its Vice President for

Academic Affairs, ATTY. RODERICK P. VERA with principal address in Biglang Awa,
Caloocan City and hereinafter referred to as the FIRST PARTY.

The _____Company Name___________________ represented by its

______________Name of Signatory_________, with principal address in
___________Company Address__________________ and herein after referred to as the
SECOND PARTY.

The student trainee, represented by _____Student Name___________, 4th year Bachelor of

Science in Accounting Technology student enrolled in University of Caloocan City.

WITNESSETH:

WHEREAS, the University of Caloocan City is a higher learning institution offering

different certificate/baccalaureate programs and desirous of producing ideal and competent
professionals of the country.

WHEREAS, part of the program of the university is to strengthen the capabilities of its
students by providing top-notch on –the – job –training with its partner institutions to develop the
skills highly needed by the industry;

WHEREAS, the UNIVERSITY OF CALOOCAN CITY and the

______________Company Name____________________ are partners in developing the youths
into productive individuals.

NOW THEREFORE, for and in consideration of the foregoing premises, hereby agree the
following terms and conditions of the Memorandum of Agreement.

FOR THE FIRST PARTY:

1. Submit a list of student trainees taking particular courses requiring training a week after
the opening of each semester, the course requirements and specific facility/equipment
required for the conduct training.
2. Orient the student trainees regarding the rules and regulation of the On the Job Training
3. Issue an official endorsement vouching for the well-being of the on-the-job student trainees
which shall be used by the COMPANY for processing the application of the student-trainee
FOR THE SECOND PARTY:

1. Provide necessary compliment to secure the safety of the student trainees during their stay
in the company.
2. Allow the student trainees an access to information regarding the company, subject to the
company’s confidentiality agreement.
3. Provide the student trainees with the best mentors to enhance their experience in using the
different facilities/equipment.
4. Submit performance evaluation reports of student trainees.

This AGREEMENT shall take effect immediately upon signing by all parties concerned
and shall remain in force unless sooner terminated by mutual consent.

IN WITNESS WHEREOF, parties of the agreement hereunto affix their signature this
