Congratulations on successfully participating in the Health Insurance Portability and Accountability (HIPAA) & Information Security
training, levels I (Privacy) & II (Security) reviews. This certificate of recognition is good for one (1) year from the date as referenced
above. As long as you are an employee or student at CMU, or contracted business associate, participating in or providing services for
departments or areas designated as HIPAA covered entities, an annual re-review and quiz is required. A re-review will be indicated sooner
should HIPAA regulations, CMU policy, or departmental procedures significantly change.

The training and test included the addressing of the following key points:

HIPAA Basics I ±

o What is HIPAA and how does it apply to this office or department?
o Introduction of the CMU HIPAA Team & CHP Privacy Officer±Explanation of roles.
o Core principals relating to ensuring privacy and FERPA.

More about HIPAA, Part II ±

o Explanation of core policies and related forms pertaining to privacy, confidentiality and information security.
o Medical record management, including (but not limited to):

o Authorization for release of protected health information (PHI) o Patient amendment of the medical record

o Followed by the successful administration of a test for validating full understanding of HIPAA and Information Security (HITECH)
