AFFIDAVIT

I, JHORIEL M. SANTISTEBAN, of legal age, single, Filipino, and presently

residing at B22L34 Topaz St., Citation Homes, Bahay Pare, Meycauayan City, Bulacan,
after having been duly sworn to in accordance with law, do hereby depose and say that:

1. I am the true and registered owner of a parcel of land located at Citation

Homes, Bahay Pare, Meycauayan Bulacan, and covered by Transfer Certificate Title No.
040-2014013051;

2. I acquired the property through “Sale with Assumption of Mortgage,” and

said acquisition was entered as entry no. 2014009248, dated 28 May 2014. Said entry
caused the cancellation of previous title, TCT No. T-147980 in lieu of Transfer Certificate
of Title No. 040-2014013051 which is now under my name JHORIEL M.
SANTISTEBAN;

3. However, upon the issuance of the new Transfer Certificate of Title No.
040-2014013051, I noticed that there were two annotations in the new title, which should
not have been annotated therein, particularly Entry Nos. 44609 and 44610, pertaining to
the mortgage of the property in favor of Filinvest Development Corp.;

4. The said entries are deemed cancelled by the fact of my redemption of

the said property, as entered in the cancelled Certificate of Title No. T-147980,
particularly Entry No. 2014001763, Date of instrument: 20 November 2013; Date of
Inscription: 30 January 2014 at 8:50 A.M;

5. Hence, the said mortgages to Filinvest, as stated in the said entries

44609 and 44610 should not be included in the new Transfer Certificate of Title No. 040-
2014013051;

6. In order to reflect the true and absolute ownership of the property by the
new registered owner, I JHORIEL M. SANTISTEBAN, respectfully request the Registry
of Deeds of Meycauayan, Bulacan, to remove the Memorandum of Encumbrances Entry
Nos. 44609 (M) and 44610 (M) in the Transfer Certificate of Title No. 040-2014013051;

7. I am executing this Affidavit to attest to the truth of the foregoing, and to

be able to clear any discrepancy and/or confusion, which may have been caused by the
inadvertent annotation of Entries 44609 (M) and 44610 (M) in the said Transfer
Certificate of Title No. 040-2014013051.
