Abstract
Purpose: Allergic conjunctivitis (AC) is associated with itching, redness, tearing, pain, and burning

release mediators. The purpose of this study was to compare topical olopatadine and ketotifen in terms of
effectiveness and safety for the management of AC.
Methods: Patients clinically diagnosed with AC were randomized into two groups of 60 patients each and
received either topical olopatadine HCl 0.1% or ketotifen fumarate 0.025%. They were followed up on the

PENDAHULUAN
Konjungtivitis alergi adalah kondisi atopik mata, yang berhubungan dengan
gatal, mata merah, berair, nyeri, sensai terbakar, dan sensasi benda asing. [1,2]

mengakibatkan hilangnya produktivitas.[3] Konjungtivitis alergi dapat mengenai
anak-anak dan orang dewasa, sering disertai dengan penyakit alergi lainnya,
seperti asma, dermatitis atopik, atau alergi makanan.[4] Menurut International
Ocular Inflammation Society (IOIS), konjungtivitis alergi dapat dibagi menjadi
konjungtivitis alergi musiman dan konjungtivitis alergi perennial. Ini juga
termasuk keratokonjungtivitis atopik, keratokonjungtivitis vernal, giant papillary
konjungtivitis, dan dermato-konjungtivitis kontak, yang berbeda dalam
manifestasinya, perjalanan klinis, dan terapi.[5]

METODE

institusi komite etik. Pasien baik dengan jenis kelamin laki-laki ataupun
perempuan yang berusia lebih dari 8 tahun, secara klinis didiagnosa dengan
konjungtivitis alergi musiman oleh dokter spesialis mata dan bersedia melalui
inform consent/persetujuan tertulis telah direkrut.

Pasien dengan bakteri, chlamydial, virus, giant papillary, phlyctenular,
purulen, dan konjungtivitis membranosa telah dieksklusikan. Adanya sindrom
mata kering, blepharitis, uveitis, keratitis, trauma okular, atau riwayat operasi
mata yang dilakukan dalam 3 bulan terakhir menyebabkan pasien juga
dieksklusikan. Kriteria eksklusi lainnya termasuk ablasi retina, retinopati diabetik,
dan penyakit retina progresif; menerima kortikosteroid baik sistemik maupun
topikal dan imunosupresan; dengan penggunaan stabilisator sel mast, NSAID,
antihistamin dalam bulan sebelumnya; dan riwayat hipersensitivitas terhadap
olopatadine dan ketotifen. Wanita hamil dan menyusui juga dieksklusikan.
Pasien secara acak dimasukkan ke salah satu dari dua kelompok; satu
kelompok menerima dua tetes olopatadine HCl 0,1% (Winolap, diproduksi oleh
Sun, Avesta) dua kali sehari di kedua mata, sementara yang lain diobati dengan
dua tetes ketotifen fumarate 0,025% (Albalon, diproduksi oleh Allergan) empat
kali sehari. Mereka diminta memantau setiap harinya untuk mencatat waktu
pemberian obat mereka. Berikut ini dicatat pada pemeriksaan klinis. Gejala dan
tanda pasien dinilai menggunakan skala, dengan skor mulai dari 0 hingga 16.[8]
Setiap QOL pasien dinilai menggunakan kuesioner yang terdiri dari 15
pertanyaan, dengan skor mulai dari 0 hingga 90.[9]
Penilaian skor dari tanda dan gejala dilakukan pada kunjungan pertama
(dasar), hari ke-4, dan hari ke-15. Jika tanda-tanda klinis menetap, para peserta
juga dievaluasi pada hari ke 30. Kuesioner QOL diberikan pada hari ke-1, ke-4,
dan ke-15. Para peserta diminta untuk membawa buku harian mereka ke setiap
kunjungan untuk memeriksa kepatuhan. Perbaikan gejala dan tanda pasien
dievaluasi. Reaksi obat yang merugikan dicatat pada setiap kunjungan. Biaya
dihitung berdasarkan jumlah yang dikeluarkan oleh pasien untuk kesembuhan
sempurna konjungtivitis alergi.
ANALISIS STATISTIK
Ukuran sampel yang diperlukan untuk mendeteksi perbedaan rata-rata dari
0,35 pada skor gatal pada hari ke-GHQJDQXNXUDQHIHNĮ-error 5%, kekuatan
80%, dan tingkat dropout 10%, adalah 32 pasien di masing-masing kelompok.
Data demografis dinyatakan sebagai rata-rata ± standar deviasi. Data kontinu di

dalam dan di antara kelompok dianalisis dengan menggunakan uji t berpasangan
dan tidak berpasangan. Skor konjungtivitis alergi dan QOL di dalam dan di antara
kelompok masing-masing dianalisis menggunakan R̻ ANOVA dan uji t tidak
berpasangan. Data kategori dianalisis menggunakan uji Chi-square. Signifikansi
statistik ditetapkan pada P <0,05.
HASIL
Sebanyak 120 pasien termasuk dalam penelitian ini. Dari jumlah tersebut, 55
pasien di kelompok A dan 54 pasien di kelompok B telah menyelesaikan
penelitian [Gambar 1]. Ada 67 laki-laki dan 53 peserta perempuan. Seperti yang
ditunjukkan pada Tabel 1, rincian demografis antara kelompok-kelompok
sebanding. Riwayat konjungtivitis alergi sebelumnya terdapat pada 42 (70%) dan
34 (57%) pasien masing-masing dalam kelompok A dan B. Faktor yang
mengganggu untuk konjungtivitias alergi adalah variasi musiman (musim panas;
masing-masing 25 dan 16 pasien) dan debu (masing-masing 17 dan 18 pasien)

Baseline skor konjungtivitis alergi individu dan total sebanding antara kedua
kelompok. Tidak ada pasien yang mengalami kemosis. Dibandingkan dengan
baseline, pasien yang menerima baik olopatadine atau ketotifen menunjukkan
penurunan yang signifikan (P = 0,001) pada skor konjungtivitis individu dan total,
pada hari ke-4 dan ke-15 [Tabel 2]. Analisis antar kelompok menunjukkan bahwa
pada hari ke-4, gatal, mata berair, merah, dan skor konjungtivitis alergi total,
tetapi tidak papil, telah berkurang secara signifikan dengan olopatadine (P =
0,001). Skor pembengkakan kelopak mata telah berkurang menjadi nol dengan
kedua obat pada hari ke-4 [Tabel 2]. Pada hari ke-15, terdapat penurunan yang
signifikan (P = 0,001) pada mata gatal, berair, papil, dan skor konjungtivitis alergi
total pada pasien yang menerima olopatadine. Skor pembengkakan kelopak mata
dan mata merah, bagaimanapun, adalah nol dengan kedua obat.

Baseline Skor QOL sebanding antara kelompok. Dibandingkan dengan
baseline, ada penurunan skor QOL yang signifikan pada hari ke-4 dan ke-15 pada

Reaksi yang merugikan dilaporkan pada 10% dan 18% pasien, masing-
masing dengan olopatadine dan ketotifen [Tabel 4]. Karena setiap pasien
memerlukan satu vial olopatadine HCl, biaya per pasien adalah 84 rupee. Satu vial
ketotifen fumarate adalah 57 rupee, dan karena 32 pasien membutuhkan dua vial,
biaya untuk pasien ini adalah 114 rupee.
DISKUSI
Konjungtivitis yang disebabkan oleh non-infeksi stimuli termasuk
konjungtivitis alergi musiman dan konjungtivitis alergi perennial, yang
mempengaruhi individu dari semua kelompok umur, terutama anak-anak. Gejala

konjungtivitis alergi musiman, yang dapat mempengaruhi kedua mata, diperberat
karena variasi musiman atau adanya debu. Gejala dan tanda-tanda okular
termasuk mata gatal, berair, konjungtiva hiperemis, pembengkakan kelopak mata,
kemosis, dan sensasi benda asing,[10,11] yang jika tidak ditangani dapat menjadi
lebih buruk. Hal ini selanjutnya dapat menyebabkan ketidaknyamanan yang akan
mempengaruhi kualitas hidup. Dibandingkan dengan kortikosteroid, pada kondisi
ini lebih baik diobati dengan stabilisator sel mast karena efek samping yang lebih
sedikit.
Dalam penelitian ini, dari 120 pasien yang didiagnosa secara klinis dengan
konjungtivitis alergi, 56% adalah laki-laki dan 44% adalah perempuan. Rasio laki-
laki dan perempuan adalah 1,2: 1. Sebuah penelitian yang dilakukan di Ghana
menunjukkan bahwa persentase wanita (61,8%) dengan konjungtivitis alergi lebih
tinggi.[12] Penelitian lain yang dilakukan di Nigeria, termasuk 150 siswa (usia 5±
15 tahun), melaporkan bahwa anak perempuan terdiri dari 59% dari kohort.[13]
Peningkatan prevalensi pada wanita telah dikaitkan dengan perubahan hormon
wanita.[12,13] Dua penelitian lain, bagaimanapun, melaporkan bahwa 75,8% dan
63,28% pasien dengan konjungtivitis adalah laki-laki.[14,15]
Pasien dalam penelitian ini memiliki riwayat konjungtivitis alergi
sebelumnya. Meskipun persentase dari pasien melaporkan bahwa gejala
konjungtivitis alergi memberat selama musim panas dan terpapar debu lebih
tinggi pada kelompok olopatadine, perbedaannya tidak signifikan. Sebuah studi

oleh Palmares dkk menunjukkan bahwa sekitar 85% dari kohort memiliki
episode konjungtivitis alergi sebelumnya dan 16% dari kohort memiliki asma
bronkial. Namun, dalam penelitian ini, pasien dengan asma bronkial dikeluarkan
karena mereka menerima obat seperti steroid sistemik atau inhalasi dan
stabilisator sel mast, yang akan mengintervensi pengobatan dalam penelitian ini.
Sebuah penelitian di Nigeria yang dilakukan pada anak-anak sekolah dasar
melaporkan bahwa penyakit ini lebih umum terjadi selama musim harmattan
(angin perdagangan Afrika Barat yang kering dan berdebu) karena adanya debu
dan serbuk sari di atmosfer.[12]

Gatal merupakan keluhan yang paling umum dari pasien. Skor konjungtivitis
alergi individu dan total sebanding antara kelompok pada baseline. Pengobatan
dengan olopatadine 0,1% secara signifikan mengurangi skor gatal selama masa
follow-up dan pasien benar-benar bebas dari gejala pada hari ke-15. Temuan
serupa dilaporkan dalam penelitian yang dilakukan di Hungaria, yang mencakup
orang dewasa dan anak-anak; skor gatal berkurang masing-masing dari 1,6
menjadi 0 dan dari 2,5 hingga 0,2, pada akhir 14 hari.[17] Penelitian ini juga
menyimpulkan bahwa penggunaan olopatadine pada anak-anak aman.[17] Dalam
tantangan alergen konjungtiva pada manusia, olopatadine 0,1% dan 0,2%
dibandingkan, dengan tanpa perbedaan yang signifikan dalam skor gatal antar
dosis.[18] Selain itu, penurunan yang signifikan diamati pada 24 jam, dengan kedua
dosis, ketika dibandingkan dengan plasebo.[18] Sebuah studi pada pasien di Jepang
dengan konjungtivitis alergi musiman menunjukkan bahwa pretreatment dengan
olopatadine secara signifikan mengurangi skor gatal. Para penulis mengaitkan
temuan ini dengan kandungan stabilisasi sel mast pada obat.[19] Olopatadine 0,1%
juga secara signifikan mengurangi skor mata berair pada semua kunjungan follow-
up, bila dibandingkan dengan natrium kromoglikat 2%.[20] Olopatadine juga
secara signifikan mengurangi mata merah di kohort pasien ini. Hasil serupa
[21]
dilaporkan oleh Yaylali dkk ketika mereka menggunakan olopatadine dan
ketorolak untuk mengobati konjungtivitis alergi musiman.
Pembengkakan kelopak mata menyebabkan banyak ketidaknyamanan pada
pasien. Dibandingkan dengan cromolyn sodium 2%, olopatadine 0,1%
mengurangi pembengkakan kelopak mata pada hari ke-4 pengobatan.[22] Papil
juga merupakan tanda konjungtivitis alergi, yang secara signifikan berkurang pada
hari ke-4 dari penggunaan olopatadine. Penurunan lengkap dalam skor tanda dan
gejala hanya diamati pada 15% pasien pada hari ke-4, tetapi pada hari ke-15
penurunan diamati pada semua pasien.
Penurunan skor konjungtivitis alergi individu dan total diamati pada setiap
kunjungan follow-up pada pasien yang menerima ketotifen. Skor berkurang
menjadi nol pada hari ke-15 hanya pada 46,6% dari kohort pasien. Studi lain
menunjukkan bahwa ketotifen 0,05% mengurangi gatal, rasa menyengat, dan mata

berair setelah 10 hari pada 60-80% pasien.[23] Ketika dibandingkan dengan
levocabastine dan plasebo, ketotifen merupakan yang paling efektif selama empat
hari pertama dalam mengurangi skor gatal, mata merah, dan berair.[24] Meskipun
pembengkakan kelopak mata juga berkurang, ini tidak signifikan secara
statistik.[24] Pengamatan penulis adalah bahwa pengurangan skor papil
membutuhkan waktu lebih lama dengan ketotifen, dengan hanya 63% dari kohort
pasien yang menunjukkan pengurangan lengkap pada hari ke-15.
Dibandingkan dengan ketotifen, olopatadine secara signifikan mengurangi
gatal, mata berair, merah dan skor total konjungtivitis alergi pada hari ke-4, dan
pembengkakan kelopak mata dan papil pada hari ke-15. Ini menunjukkan bahwa
olopatadine memberikan perbaikan gejala lebih cepat daripada ketotifen. Temuan
serupa dilaporkan dalam penelitian lain di mana 42,5-62,5% pasien yang
menerima olopatadine menunjukkan perbaikan tanda gejala pada 30 menit
dibandingkan dengan 20-27,5% yang menerima ketotifen.[25] Selanjutnya, pada
hari ke 7, olopatadine mengurangi gejala hingga 80-87,5%, sementara ketotifen
mengurangi gejala sebanyak 60 75%.[25] Dua penelitian lain telah melaporkan
bahwa olopatadine 0,1% lebih efektif daripada ketotifen.[26,27]
QOL, parameter lain yang dinilai, juga ditingkatkan dari baseline pada kedua
kelompok ke tingkat yang sama. Sebuah studi yang dilakukan oleh Scoper dkk [28]
menunjukkan bahwa pasien yang menerima olopatadine 0,2% memiliki
peningkatan yang signifikan dalam kualitas hidup. Olopatadine juga lebih disukai
oleh pasien di tempat lain.[29]
Dalam penelitian saat ini, total 10% dan 18% pasien melaporkan reaksi yang
merugikan dengan olopatadine dan ketotifen, masing-masing, yang paling umum
adalah sakit kepala, diikuti oleh sensasi terbakar di mata. Dalam penelitian lain,
yang termasuk 100 pasien, 98% pasien yang menerima ketotifen melaporkan
sensasi terbakar di mata.[29] Sensasi menyengat di mata juga diamati pada 22,5%
pasien yang menerima ketotifen.[25] Sebuah studi memantau reaksi obat yang
merugikan (ADRs) untuk obat yang berbeda, menemukan bahwa masing-masing
olopatadine 0,1% dan ketotifen menyebabkan ADR pada 4,65% kasus.[30] Dalam
penelitian ini, mmasing-masing 4 dan 8 pasien, yang diobati dengan olopatadine

dan ketotifen, melaporkan sakit kepala, sementara masing-masing 2 dan 3 pasien,
melaporkan sensasi terbakar.
Dalam penelitian ini, olopatadine memberikan perbaikan tanda dan gejala
yang lebih cepat dan biaya yang lebih efektif dibandingkan ketotifen.
Kesimpulannya, konjungtivitis alergi dapat diobati dengan antihistamin
topikal, stabilisator sel mast, NSAID, dan steroid. Temuan dari penelitian ini
mengungkapkan bahwa pasien yang menerima olopatadine HCl 0,1% memiliki
perbaikan gejala konjungtivitis alergi ebih cepat daripada ketotifen fumarate
0,025%. Pada akhirnya, kedua obat meningkatkan kualitas hidup hingga tingkat
yang sama.
