Important Message From The Elementary School Nurse

Dear Parent/Guardian:

Welcome to the 2018-2019 school year! I am so excited to join your child on this journey. This will be my second
year as the Elementary school nurse for the Litchfield School District. I am available during regular schools days
from 8:00 AM to 3:30 PM. If you have a question or concern about your child’s health, please contact me through
your child’s school or through e-mail at kkirby@litchfieldpanthers.org.

State law requires that all students must meet requirements regarding physical, dental, and eye examinations
along with immunizations. Any student who is entering Litchfield CUSD #12 as a Pre-K, Kindergarten, or transfer
student from out-of-state, will be required to have a physical prior to the start of school. Please note, the physical
form also includes a health history section that MUST be filled out and signed by the parent to make the physical
complete.

All students entering Kindergarten and 2nd grade are required to have a dental exam. The dental exam has to be
dated between November 15, 2017 and May 15, 2019 in order to be accepted. The dental exam must be
completed by May 15, 2019 and brought to the school office.

All students must meet the immunization and lead screen requirements prior to the start of school. All children
entering Pre-K and Kindergarten are required to provide their most recent lead screen date and result. The
Montgomery County Health Department has an immunization clinic in Litchfield at 317 West Union the first
Thursday of each month from 2:30 – 5:00 PM and in Hillsboro on the third Thursday of each month from 2:30-5:00
PM at the main office.

All students entering Kindergarten or enrolling for the first time in an Illinois school are required to have an eye
exam by a physician licensed to medicine in all its branches that provide eye examinations or a licensed
optometrist within the previous year before October 1, 2018. In other words the exam has to dated between
October 15, 2017 and October 1, 2018. Forms are currently available for the Optometrist to complete at your
child’s school office or at http://www.idph.state.il.us/HealthWellness/EyeExamReport.pdf.

The State of Illinois regulates the administration of medication in schools. This means that I cannot dispense
medication to your child unless a School Medication Authorization Form is signed by you and your physician. This
also includes Tylenol (acetaminophen), Advil (ibuprofen), and other over the counter medications. Medication
Authorization Forms are available through the school office or on the district’s website.

If your child will be taking medication at school, the medication must be brought to your child’s school office by a
parent or guardian and be in the most recent, original prescription bottle. The medication will not be accepted if it
is in anything other than the original prescription bottle or if it is brought in by your child.

If your child has any severe allergies, history of seizures, or asthma, an action plan needs to be filled out by the
doctor and signed by you. These forms are available at your school office.
