AFFIDAVIT OF SERVICE

I, Berlin Labrador, of legal age, single/married and with office address at Sayre
Highway, Hagkol, Valencia City, Bukidnon, after having been duly sworn in accordance
with law, hereby depose and state that:

1. I am the Branch Manager of Cebu Sendai Motor Sales, Inc. (hereafter, the
“Company”) As such, I was tasked to personally served the Demand Letter to
the Company’s delinquent customer, Mr. Diego P. Verador (hereafter,
“Diego“).

2. Diego P. Verador (hereafter, “Diego”) and Leonora M. Limansag (hereafter,

“Leonora”, the co-maker) whose account became delinquent starting June
15, 2015 and his last Monthly Amortization payment was made on March 28,
2018. Thus, in order to notify Leonora of such fact, our Credit and Collection
Manager, Mr. Elpidio R. Pizarra, prepared a demand letter addressed to her
dated January 12, 2018.

3. In order to personally serve the same, I scheduled a trip to visit Leonora at her
residence at Purok 5, Kulasihan, Lantapan, Bukidnon last February 21, 2018.

4. When I served the said demand letter, it was received by Maricel Verador
(wife of Diego), who personally received the same. I am certain of this since I
have met her even before February 21, 2018 and I am certain that she was
the same person to whom I served the demand letter. Before handling to her
said demand letter, I even talked and asked her to settle their obligations
with the Company.

5. As a matter of procedure, I requested Maricel Verador to sign on the

receiving copy of the demand letter. Such demand letter was personally
received by Maricel Verador as signified by her signature at the bottom left
of the said demand letter.

6. Thereafter, I left the original of the demand letter to Maricel Verador and
brought back to the office the receiving copy thereof.

7. I execute this Affidavit to attest to the truth of the foregoing and for
whatever legal purpose it may serve.
