EQUIPMENT RENTAL AGREEMENT FORM

Rental fee: P500

I, the undersigned, agree to return all of the equipments issued to me to the same
location I picked it up at, in the same condition as when I received it on the date issued.
Any damage arising from mishandling and improper operation of airsoft guns during
the whole course will be charged to the person using the it. I agree to pay any additional
balance due to cover the full replacement cost for lost/stolen or damaged item/s while
in possession thereof. Failure to return any equipment issued will be regarded as theft
and will be reported to the proper authorities. I agree to check all the equipments issued
to me before I use it and will not use the same if any part/s thereof are damaged or
missing. By signing this agreement I understand and adhere to all the above conditions.
